Our team of more than 2,000 employees speak 22 languages and are dedicated to offering hope and unlocking the full potential of individuals and families through culturally responsive, trauma-informed, research-based services for individuals and families from birth to older adults.</p> <p>Who We Serve:</p> <p>Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families. Our diverse staff provides culturally and linguistically relevant services in over 22 languages to our region's culturally and ethnically diverse populations. We provide services in 18 counties across California.</p> <p>The Position:</p> <p>The Mental Health Worker/ Case Manager facilitates the implementation of individualized Service Plans, provides client advocacy services and case management strategies.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Conducts strengths-based, culturally competent and family-centered mental health rehabilitation services and/or case management assessments gathering information from family, consumer, significant others and involved agencies. </li><li>Encourages clients to meet needs through matching strengths with tasks. </li><li>Provides rehabilitative services to clients and/or their families which might include assistance in restoring or maintaining a client's functional skills, daily living skills, social skills, and linking up to needed community services and support resources. </li><li>Completes Service and Coordination Plans, progress notes, and other DMH required documentation. </li><li>Provides outreach, advocacy, and rehabilitative services needed for ongoing cases and in crisis intervention. </li><li>Provides case management, which might include obtaining client information, identifying needs and problems, linking clients and their families to appropriate community resources. </li><li>Works with the treatment team to provide appropriate coordinated care services to clients and their families and to address the client needs advocating for stabilizing services and connecting the consumer to beneficial resources within the community. </li><li>Works on a multicultural team sensitive to the cultural and linguistic needs of the clients and families served. </li><li>Works in conjunction with community providers to assist clients in meeting goals. </li><li>Connects the client and family to community resources maximizing information from the strengths-based assessment. </li><li>Provides services in the field, at home, in the school and/or in the community to promote stabilization and consistent with program and funding source contractual requirements. </li><li>Under the direct supervision of the Program Director and team therapist, assists in the identification of triggers to escalation, and provides suggestions to the treatment team. </li><li>Considers substance abuse, healthcare, co-occurring disorders and/or housing or employment, as appropriate, in treatment plans. </li><li>Completes billing expectations and provides services consistent with program needs. </li></ul> <p>EDUCATION and/or EXPERIENCE/POSITION REQUIREMENTS:</p> <ul> <li>Bachelor of Arts degree or two (2) years related experience with chronically mentally ill populations and/or in social services related field. </li><li>Chemical dependency experience required for positions in dual diagnosis programs, preferred in other programs </li><li>Excellent written and verbal communication skills. </li><li>Experience using Microsoft Office Suite and Electronic Health Record. </li><li>Bilingual skill in the appropriate language for the Clinics' clients is highly preferred. </li><li>Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), background clearance. </li><li>Must possess a valid CA driver's license and insurance with two years experience and maintain an insurable driving record under the agency's liability policy. </li><li>------------------------------------------------------------------ </li></ul> <p>Equal Opportunity Employer</p> <p>We will consider for employment qualified Applicants with Criminal Histories in a manner consistent with ordinance 184652 Sec.189.04 (a) and San Francisco Police Code, Article 49.
