<p>Location: DTLA Hotel, Downtown Los Angeles, CA</p> <p>About the Program:</p> <p>The San Julian Oasis (SJO) Recuperative Care Program is an interim housing program that provides bed space for eligible patients. To qualify, patients must be unhoused, have an acute medical condition, and be medically and physically stable. This respite program offers medical care, case management, personal care, three meals per day, access to long-term housing resources and placement, and reunification services for clients who have lost contact with their family or support network.</p> <p>Position Purpose:</p> <p>The Client Aide Worker and Peer Advocate Supervisor is an integral member of a multi-disciplinary team delivering health and social services to clients of the San Julian Oasis Recuperative Care program. This position plays a key role in supporting client engagement, retention, and daily living activities within The Oasis. Working closely with the behavioral health team, the Supervisor helps implement supportive, client-centered interventions and delivers skills-building guidance to the team.</p> <p>The role also includes assisting clients with navigating activities of daily living (ADLs), both onsite and in the community; ensuring support is available 24/7. The Client Aide Worker and Peer Advocate Supervisor collaborates with the medical care team to coordinate and escort clients to medical and psychiatric appointments. Additionally, they are responsible for training incoming Peer Advocates, supporting daily operational needs, and maintaining the weekly cleaning schedule to ensure a safe and supportive environment.</p> <p>Principal Responsibilities:</p> <ul> <li>Establish a rapport with clients and express a concern for their well-being. </li><li>Provide support to clients and motivate individuals to adhere to behavior modification program. </li><li>Assist with maintaining complete updated, confidential file on the status of the client and documentation of their progress in changing personal behaviors. </li><li>Contact clients on a daily basis to assess support needs </li><li>Deliver education and skills-building information based on program activities and curriculum. </li><li>Provide supportive communication regarding incarceration, substance abuse, mental health, and other risk behaviors. </li><li>Assist in collecting program data and processing program forms on an ongoing basis. </li><li>Performs job functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities </li><li>Observes and documents client response to care </li><li>Reports and records observations of client conditions </li><li>Assist with meals as needed </li><li>Will assist clients on continual growth by aiding them in their range of motion exercises </li><li>Assist partners within the client care team to support client health and safety </li><li>Assists in promoting client safety and environmental cleanliness/safety </li></ul> <ol start="9"> <li>Documents pertinent client information, following established policy and procedures: </li></ol> <ul> <li>Documents accurately in the DHS electronic medical record or other methods as directed, regarding observations of client response to care or physical conditions </li><li>Documents all data clearly and thoroughly- using name and title, date and time </li><li>Follows all DHS requirements for logging/documenting/reporting client medical status </li></ul> <ol start="10"> <li>Maintains confidentiality at all times: </li></ol> <ul> <li>Discusses any client information only with appropriate personnel when related to the care being provided, </li><li>Maintains confidentiality for all documents that contain client identifier information, </li><li>Observes and respects confidentiality of information in regards to fellow employees. </li></ul> <ol start="11"> <li>Responsibility and accountability for adherence to organizational and department standards and policy: </li></ol> <ul> <li>Observes department guidelines concerning absences and reports absences to Program Director/Asst. Director in accordance with departmental procedures, </li><li>Self-identifies potentially unsafe systems/processes/situations and oversees that staff comply with safety measures, </li><li>Ensure that staff observes department guidelines to conduct personal business during breaks and lunch hour, </li><li>Projects a positive professional image, and stress the importance of staff adhering to organizational dress code. </li></ul> <ol start="12"> <li>Participates in organizational programs/committees as assigned: </li></ol> <ul> <li>Ensue the participation of staff in meetings, for operations and training. </li><li>Oversees and participates in all safety programs, which may include assignment to an emergency response team, </li><li>Performs other duties and tasks as may be required or assigned by Program Director or Assistant Director. </li></ul> <ol start="13"> <li>Team Work and Interpersonal Skills: </li></ol> <ul> <li>Demonstrates courtesy, mutual respect and compassion in dealing with staff, </li><li>Initiates and presents positive solutions to problems and encourages staff to take the initiative in problem solving. </li><li>Demonstrates effective working relations and works effectively as the supervisor to challenge the team to meet goals and objectives. </li><li>Demonstrates a willingness to accommodate requirements or changing priorities in the workplace. </li></ul> <ol start="14"> <li>Performs other duties as assigned. </li></ol> <p>Requirements:</p> <ul> <li>Minimum High School diploma or equivalency </li><li>Minimum one-year experience in a health care setting preferred. </li><li>Current BLS Certificate from the American Heart Association </li><li>Bilingual English/Spanish a plus </li><li>Previous experience working with homeless persons. </li><li>Strong ability to communicate with homeless individuals who are actively participating in substance abuse and other factors related to homelessness. </li><li>Ability to travel and arrive to sites on time within the community. </li><li>Desire to work with homeless persons </li><li>All JWCH, Wesley Health Centers workforce members are recommended to be fully vaccinated and boosted against COVID-19. </li></ul> <p>Employee Benefits:</p> <p>At JWCH Institute, Inc., we believe in taking care of those who take care of others.
