<p>Job Description Summary</p> <p>Nurse Practitioner, Specialty - Medical Office, Nephrology</p> <p>How will you make an impact & Requirements</p> <p>Work Site Locations: Upland, West Covina and Apple Valley. May be required to travel to other clinics for business need.</p> <p>Work Shift: 5 Days a Week, Monday - Friday: 8:00 am - 5:00 pm</p> <p>The Nurse Practitioner Specialty is responsible for providing for the health care needs of patients in collaboration with a physician, through advanced assessment and identification of health states including abnormal conditions, diagnosing, treatment therapies and/or evaluation. Performs care in the specialty area of nephrology and patients on dialysis.</p> <p>How will you make an impact:</p> <ul> <li> <p>Utilizes evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines and physician instructions.</p> </li><li> <p>Acts independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations.</p> </li><li> <p>Acquires patient health histories and evaluates patient, performing physical examinations.</p> </li><li> <p>Orders, interprets and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs.</p> </li><li> <p>Discusses case with physician.</p> </li><li> <p>Formulates and documents care plan.</p> </li><li> <p>Prescribes medication, therapies and/or other forms of treatment.</p> </li><li> <p>Evaluates the patient's response to therapy.</p> </li></ul> <p>Minimum Requirements:</p> <ul> <li> <p>Requires an MS in Nursing and minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.</p> </li><li> <p>Active Nurse Practitioner Board Certification</p> </li><li> <p>Current, unrestricted RN license and NP license in the state of California is required.</p> </li><li> <p>Certification in a specialty or minimum of 1 year experience in the specialty area of nephrology.</p> </li></ul> <p>Preferred Skills, Capabilities and Experiences:</p> <ul> <li> <p>Bilingual communication skills in English and Spanish.</p> </li><li> <p>Experience in either Primary Care or an Internal Medicine setting with adults and geriatric patient population.</p> </li></ul> <p>Compensation: $132,756K - $165,945K & bonus eligible</p>
