<p>Summary:</p> <p>Under the supervision of the Director of Clinic Operations, the Mammogram Case Worker ensures that patient's enrolled in the cancer Detection Program are scheduled and processed for Clinical Breast Exams (CBE), screening and diagnostic mammograms, and follow- up specialty tests as orders by assigned providers. Assist with processing patient for their mammogram appointments. Maintain patient mammogram results in a monthly format for annual mammography FDA/DHS audits.</p> <p>Duties and Responsibilities:</p> <ul> <li>Schedule appointments for CBEs and screening mammogram at Arroyo Vista Family Health Center. </li><li>Schedule Diagnostic mammograms at off-site mammogram locations. </li><li>Coordinate appointments for specialty diagnostic procedures and follow-up and treatments of breast cancer at various off-site mammogram locations based on patient's health plan. </li><li>Enroll uninsured patients in the Every Women Counts (EWC) and Breast and Cervical Cancer Treatment Program (BCCTP) as needed. </li><li>Coordinate transportation services, translation services, and provide resources to patients as needed. </li><li>Complete and maintain documentation of all referral authorization status, appointment information, missed appointments, phone communication, and correspondence in patient's electronic health record (EHR) NextGen and i2! tracking system. </li><li>Perform daily recalls for CBE, mammogram, and follow-up appointment. </li><li>Follow-up on all broken mammogram appointments and document follow-up in i2! Tracking, and patients EHR. </li><li>Scan all CBE forms EWC/BCCTP eligibility forms, mammogram reports, progress notes, completed case management and certified letters into patient's in HER. </li><li>Assist with coordination and retrieval of previous mammogram films for comparison studies. </li><li>Prepare for on-site mammogram appointments by obtaining CBE forms and any previous mammography records for review by our Mammography Technologist and Lead Interpreting Radiologist. </li><li>Submit data and final diagnosis for each breast cycle to Medi-Cal's online data collection system (DETEC) for all patients enrolled in EWC and BCCTP at www. Medi-cal.Ca.Gov (http://Medi-cal.Ca.Gov). Scan copy of DETEC form submission to patient's EHR. </li><li>Complete case management by initiating a case management encounter in NextGen, sending patients a notice of their mammogram results, and entering a recall for their return to AVFHC for annual CBE and mammogram, or 3-6 month follow-up imaging studies, if needed, in i2! Tracking system. </li><li>Scan breast ultrasound worksheet and CBE forms to Novarad and verify studies, for radiologist review. </li><li>Utilize Novarad picture archiving and communication system (PACS) to send mammogram images to our Lead interpreting Radiologist for review, obtain screening mammogram and breast ultrasound reports, onto CD format to respond to requests from off-site facilities. </li><li>Ensure that patients complete the Authorization for Release of Information form for release of mammogram films as needed. </li><li>Responsible for following all Agency safety and health standards, regulations, procedures, policies and practices. </li><li>Other Duties as assigned by immediate supervisor. </li></ul> <p>Requirements:</p> <ul> <li>Minimum six (6) months to (1) year office work experience in the medical filed. </li><li>Work Monday to Friday </li><li>High School Graduate or GED certification. </li><li>Bilingual English/Spanish 5. Excellent written and verbal communication skills. </li><li>Proficient in Microsoft Word and Microsoft Excel </li><li>Ability to work well with clinic team in a professional manner. </li><li>Ability to prioritize and organize caseload as well as use critical thinking skills to problem solve. </li><li>Knowledge of medical terminology and patient confidentiality (HIPPA) </li></ul>
