<p>JOB DESCRIPTION</p> <p>Summary/Objective</p> <p>Obtains information, insurance verification and authorization required for proper patient treatment and referrals.</p> <p>Essential Functions</p> <ul> <li>-Schedule, problem solve, communicate, and coordinate referral appointments with outside specialists. </li><li>-Track and enter all referral requests in the Referral Tracking System or EMR on a daily basis. </li><li>-Coordinate with the Patient Navigator staff to track the status of all provider reports on patients referred to hospitals, outside clinics and Specialty Providers and follow up for reports/records not received on referred patients. </li><li>-Maintain an accurate records of external referral patient's appointments that are kept, failed, and current status in the Referral Tracking System or EMR; Monitor Referral Tracking System or EMR for outstanding provider reports that are 30 days or more old, and follow-up with the appropriate provider. </li><li>Ensure that all referral forms are sent to medical records on a timely manner, after assigned actions are completed. </li><li>-Provide Intensive tracking and follow-up on all patients designated as needing Intensive Tracking; supported by a minimum of 3, if appropriate, documented efforts to re-schedule patients. (documentation in EMR / Referral tracking Data base, or others, as appropriate) </li><li>-Produce an accurate bi-weekly report of all clients designated as needing, (Intensive Tracking), which documents efforts, results, and status of each patient. </li><li>-Fax referral form to providers that do not require any records to be sent. </li><li>-Be able to process 35-45 referrals on a daily basis. </li><li>-Routinely make reminder phone calls, and send by US mail, reminder to referral patients in advance of appointment with outside provider. </li><li>-Respond to In-house provider questions, requests and concerns regarding the status of patient referrals, care coordination or follow-up status. </li><li>-Provide transportation consultation, coordination and support to clinic patients referred to specified health care facilities, (This includes clinic provided (if applicable), Insurance based support </li><li>-Answer/ respond and document all phone calls, request and questions from patients in a timely manner in the patients EMR. Calls must be accurately managed or redirect as appropriate. Be able to handle 30-45 phone calls a day. </li><li>-Routinely provide patients with other clinic and community-based resource materials as appropriate. </li><li>-Verify insurance information for referral purposes and process insurance information for referral patients. </li><li>-Performs all job functions in a professional and courteous manner. This includes answering all general phone call timely. Provide excellent customer service to internal and external clients/patients by being responsive to all inquiries in a timely manner. </li><li>-Performs job duties collaboratively with health center management and exercises good judgement. </li><li>-Fosters and promotes a culture of service excellence and accountability. </li><li>-Consistently adheres to a high standard of professional ethics; conducts self in an ethical manner and is a role model to others. </li><li>-Complies with organizational policies and procedures. </li><li>-Must act as an advocate for the patient, to insure that all efforts are made to obtain approval of the - </li><li>Provider requested referral. </li><li>-Perform all other related duties as assigned. </li></ul> <p>MEDICAL-RELATED SUPPORT</p> <p>In addition to the above-listed core job functions, these positions involve significant emphasis on all aspects of medical-related document preparation, including; transcribing dictation, summaries, reports and other medical-related correspondence and documentation.</p> <p>KNOWLEDGE, SKILLS, AND ABILITIES</p> <p>Knowledge of:</p> <ul> <li>Computer information (software and/or hardware), including word processing, spreadsheets, database, and desktop publishing. </li><li>Business correspondence format and style. </li><li>Computer operating system commands. </li><li>Departmental policies and procedures. </li><li>Medical, legal, or other specialized terminology. </li></ul> <p>Ability to:</p> <ul> <li>Deal with issues, such as client complaints and protected information, in a tactful and confidential manner. </li><li>Establish and maintain rapport with the public and staff. </li><li>Maintain composure and focus in stressful situations. </li><li>Communicate effectively in all situations. </li><li>Maintain professional appearance, conduct and appropriate attire; </li><li>Take initiative in solving problems. </li><li>Use respectful and sound judgment when approaching problems. </li><li>Write legibly and clearly. </li><li>Process and integrate a variety of complex information from multiple sources. </li><li>Use proper business English (including spelling, punctuation, capitalization, grammar, and word usage). Bilingual English and Spanish preferred. </li><li>Read, proofread and understand complex information. </li><li>Perform basic arithmetic operations to solve multiple step problems: (addition, subtraction, multiplication and division of whole numbers and decimals), compute percentages; simultaneously perform multiple tasks that require stringent attention to detail. </li><li>Code and classify information according to guidelines and principles. </li><li>Locate and retrieve materials that are misfiled or misplaced. </li><li>Sort information alphabetically and numerically. </li><li>Interpret computerized reports, tables, and charts </li><li>Good understanding of Medical Terminology </li><li>Great Communication Skills </li><li>Teamwork Orientation </li><li>Thoroughness and Attention to Detail </li><li>Follow-through, Problem Solving </li></ul> <p>Physical DemandsThis is mostly in an office setting; however, this position may require sitting for long periods of time. Walking, bending and reaching may be required as well.</p> <p>Position Type and Expected Hours of WorkThis is a part-time position. Days and hours of work are Monday through Friday, with hours of 8:00am to 5:00pm. However, these hours may be changed as the clinic standard hours may change.</p> <p>TravelTravel is sometimes required locally, and out of the area for training occasionally.</p> <p>Education and Experience</p> <ol> <li> <p>Minimum HS diploma or GED required</p> </li><li> <p>One year Referral experience in an FQHC setting preferred</p> </li></ol> <p>Other Duties</p> <p>Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
