<p>Responsibilities</p> <p>As an RN Care Coordinator at St. Bernardine Medical Center, you will play a vital role in guiding patients through their care journey, from admission to discharge and beyond. Focused on ensuring high-quality outcomes and a seamless transition across levels of care, this position emphasizes collaboration, coordination, and compassionate advocacy.</p> <p>Key Responsibilities</p> <ul> <li>Conduct comprehensive discharge planning assessments and reassess as patient needs evolve. </li><li>Collaborate with multidisciplinary teams to develop and implement patient-centered discharge plans. </li><li>Monitor patient progression of care, ensuring appropriate resource utilization and timely discharge. </li><li>Serve as a liaison between patients, families, post-acute providers, and the care team to support smooth and informed care transitions. </li><li>Advocate for each patient's values, preferences, and unique social, cultural, and language needs. </li><li>Enhance the patient experience by promoting continuity, access, and high standards of care. </li></ul> <p>This is more than a clinical role-it is a mission-driven opportunity to make a difference in the lives of patients and families, supporting the Core Values and strategic goals of St. Bernardine Medical Center. Join a team where compassion, communication, and clinical excellence work hand in hand.</p> <p>Qualifications</p> <ul> <li>Minimum two (2) years of acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of 1 year experience. </li><li>Graduate of an accredited school of nursing. </li><li>California RN license </li><li>AHA BLS card </li><li>Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used. </li><li>Able to apply clinical guidelines to ensure progression of care. </li><li>Must have critical thinking and problem-solving skills. </li><li>Collaborate effectively with multiple stakeholders </li><li>Professional communication skills. </li><li>Understand how utilization management and case management programs integrate. </li><li>Ability to work as a team player and assist other members of the team where needed. </li><li>Thrive in a fast paced, self-directed environment. </li><li>Knowledge of CMS standards and requirements. </li><li>Proficient in prioritizing work and delegating where indicated. </li><li>Highly organized with excellent time management skills. </li><li>Excellent customer service and presentation skills are a must Strong interpersonal and written communication skills are essential Demonstrated ability to apply analytical and problem solving skills Demonstrated ability to manage multiple tasks or projects </li></ul> <p>Preferred</p> <ul> <li> <p>Graduate of an accredited school of nursing (Bachelor's Degree in Nursing (BSN)) or related healthcare field.</p> </li><li> <p>At least five (5) years of nursing experience.</p> </li><li> <p>Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred</p> </li><li> <p>Knowledge of managed care and payer environment preferred.</p> </li></ul> <p>Overview</p> <p>Founded as a faith-based hospital in 1931 by the Sisters of Charity of the Incarnate Word, Dignity Health - St. Bernardine Medical Center is a 342-bed, acute care, nonprofit hospital located in San Bernardino, California.
