People live and work happy here!</p> <p>We offer competitive wage scales, tuition reimbursement, and scholarship programs.</p> <p>IND1HH</p> <p>Position Summary</p> <p>The home health care clinical coordinator plays a key role in supporting the coordination and delivery of high-quality, patient-centered care in the home setting. This position is approximately 70% office-based and 30% field-based as a treating clinician. The role includes participating in the on-call rotation and assisting with direct care as needed. This is a non-supervisory position, focused on collaboration, coordination, and compliance-not direct staff management.</p> <p>Education, Licensure & Experience</p> <ul> <li>Graduate of an accredited nursing program </li><li>Current Registered Nurse (RN) license in the State of Nebraska; ongoing CEUs required </li><li>CPR certification required (or willingness to obtain) </li><li>Minimum of two (2) years of home health experience required </li><li>Experience in long-term care or acute care preferred </li></ul> <p>Qualifications</p> <ul> <li>Must meet all qualifications to practice as an RN in Nebraska </li><li>Reasonable accommodations will be made for qualified individuals with disabilities </li><li>Must have a valid driver's license and reliable transportation </li><li>Comfortable working independently in both clinical and administrative environments </li></ul> <p>Key Responsibilities</p> <p>Admissions & Onboarding</p> <ul> <li>Assist with patient evaluations and eligibility for home health services </li><li>Support the smooth transition and coordination of new admissions </li><li>Participate in "Start of Care" visits as needed </li></ul> <p>Care Coordination & Clinical Support</p> <ul> <li>Participate in planning and coordination of patient care </li><li>Monitor and document changes in patient conditions </li><li>Communicate effectively with physicians, caregivers, and the care team </li><li>Serve as a treating clinician in the field when necessary </li></ul> <p>Office-Based Clinical Operations (Approx. 70%)</p> <ul> <li>Support clinical team operations and assist with day-to-day scheduling coordination </li><li>Perform clinical record audits, track follow-ups, and assist in quality improvement processes </li><li>Help develop or update care protocols and documentation standards </li></ul> <p>Field-Based Patient Care (Approx. 30%)</p> <ul> <li>Provide hands-on care in the home setting when needed </li><li>Support coverage for open visits or during periods of increased patient census </li></ul> <p>Compliance & Quality Assurance</p> <ul> <li>Participate in regulatory readiness, internal audits, and infection control measures </li><li>Ensure clinical practices align with agency policies and industry standards </li></ul> <p>Meetings & Communication</p> <ul> <li>Attend required team meetings and case conferences </li><li>Keep the Director of Home Health informed of patient and operational concerns </li></ul> <p>Employee Health & Wellness Program Support</p> <ul> <li>Assist with internal health screenings (e.g., TB tests, flu shots, drug testing) </li><li>Support logistics for staff wellness initiatives </li></ul> <p>Professionalism & Conduct</p> <ul> <li>Uphold HIPAA standards and patient confidentiality </li><li>Follow all policies as outlined in the Personnel Manual and Employee Handbook </li><li>Promptly report concerns related to care quality or staff conduct </li></ul> <p>Skills & Competencies</p> <ul> <li>Strong organizational and communication skills </li><li>Proficient with clinical software and general office systems </li><li>Ability to work independently, prioritize tasks, and remain calm in high-pressure situations </li><li>Familiarity with general office procedures (phones, faxing, copying, etc.) </li></ul> <p>Physical Requirements</p> <ul> <li>Must be able to stand, walk, bend, kneel, and occasionally lift up to 80 lbs </li><li>Adequate vision, hearing, and manual dexterity required for clinical care </li></ul> <p>Work Environment</p> <ul> <li>Combination of office and patient home settings </li><li>Exposure to varying home environments and infectious materials </li><li>Moderate noise level and frequent interruptions </li><li>Must be comfortable managing emotionally sensitive situations </li></ul> <p>Acknowledgment</p> <p>This job description is intended to describe the general nature and responsibilities of the position.
