<p>Please Note: This is a Temporary position through 12/31/2025.</p> <p>POSITION SUMMARY:</p> <p>The PrEP Navigator will provide patient-centered PrEP and PEP education, engagement, patient navigation and adherence counseling to PrEP and PEP patients to ensure patient engagement and retention in PrEP services.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES:</p> <ul> <li>Provide patients/clients with basic introductory education regarding PrEP/PEP. </li><li>Conduct and document screenings for PrEP/PEP eligibility including sexual and behavioral risk assessments, health literacy, and insurance coverage </li><li>Follow up with patients, as needed, to offer adherence and retention counseling based on patient's/client's needs. </li><li>Schedule individuals interested in accessing PrEP services with the Enrollment Specialist to determine the best options for care, ensuring that uninsured and underinsured clients/individuals receive appropriate education about coverage options (insurance, sliding fee, PrEP-AP) . </li><li>Coordinate and schedule patient appointments for PrEP/PEP initial and follow-up medical and non-medical visits. </li><li>Assist patients continuing PrEP and looking to reengage in services with medical appointments and additional support. </li><li>Provide PrEP navigation, as needed, for patients returning to services, tracking patient charts, and ensuring retention to required follow-up medical and/or lab appointments. </li><li>Provide PrEP/PEP education and support regarding adherence, missed dose protocols, side effects and symptoms of conversion. </li><li>Enter patient data into eCW and Red Cap, LA County's service documentation system. </li><li>Ensure proper collection, storage, and transmission of data used in reporting, research, and program evaluation. </li><li>Ensure appropriate tracking of patients and provide monthly reports on the number of outreach/in-reach efforts, referrals to APLA Health services, and progress of each patient. </li><li>Provide education about Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions; refer eligible, interested clients to primary care medical services at an APLA Health clinic. </li><li>Review and maintain up-to-date and accurate knowledge of emerging scientific research regarding biomedical prevention strategies, including PrEP/PEP, Doxy PEPand related interventions. </li><li>Participate in the identification, development, improvement and implementation of education materials and other tools that address trauma, racial/ethnic inequities and other social drivers of health and encourage healthy decisions. </li><li>Maintain highest level of client confidentiality; follow HIPAA regulations. </li></ul> <p>OTHER DUTIES MAY BE ASSIGNED TO MEET BUSINESS NEEDS.</p> <p>Qualifications</p> <p>REQUIREMENTS:</p> <p>Training and Experience:</p> <p>High School Diploma AND a minimum of 1 years of experience providing HIV education or risk reduction counseling OR one (1) year experience as a PrEP Navigator. Must demonstrate understanding of PrEP/PEP and have a strong socio-cultural identification with one or more of the target populations (Black/African American MSM, Black/African-American and Latinx cisgender heterosexual women, transgender persons, people who inject drugs and persons under 30 years of age living with HIV or at high risk for HIV). Experience in a nonprofit environment preferred; specific training/experience with HIV and other health related issues preferred. Training/experience working in HIV prevention and health education/health promotion and program development. Bilingual- Spanish speaking preferred.</p> <p>Knowledge of:</p> <p>Health disparities spectrum of HIV and STD disease; health literacy, harm reduction theory; behavior modification theory; human sexuality; substance use and abuse; motivational interviewing techniques; data management; and risk reduction theory.</p> <p>Ability to:</p> <p>Work independently; work effectively with diverse staff, volunteers and professionals; identify particular goals and objectives with respect to harm reduction; meet frequent, sometimes conflicting deadlines in a rapidly changing environment; respond with sensitivity to social inequities and diverse ethnic groups; maintain confidentiality of clients' personal health information.</p> <p>WORKING CONDITIONS/PHYSICAL REQUIREMENTS:</p> <p>This is primarily an office position that requires only occasional bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. The position requires daily use of a personal computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper.</p> <p>SPECIAL REQUIREMENTS:</p> <p>Must be able to work some nights, weekends, and occasional irregular hours.</p> <p>COVID-19 and Booster or Medical/ Religious Exemption required.</p> <p>An Equal Opportunity Employer: minority/ female/ disability/ transgender/veteran.</p>
