<p>If you want to work for a growing system of community health centers which is committed to providing quality care or patients of all ages and to the growth, education, and development of its staff, then TCC Family Health has a great employment opportunity for you.</p> <p>The Care Coordinator is responsible for providing patients with appropriate referrals to internal/external resources and agencies, coordinate specialty care for clinic patients, schedule appointments for clinic patients with a provider of a physician, serve as a liaison between the Provider, Registered Nurse (RN), Licensed Vocational Nurse (LVN), and Care Team Supervisor to ensure patients receive appropriate care. Must interact in a positive manner with staff, patients, families, and members of the community.</p> <p>ESSENTIAL JOB FUNCTIONS:</p> <ul> <li>Completes routine reports, messages, and forms </li><li>Communicates in written and verbal form, using correct grammar </li><li>Submits all correspondence within the requested time frame </li><li>Relays all messages as instructed </li><li>Organizes and files record according to established clinic procedures </li><li>Files records in chronological and alphabetical order </li><li>Effectively utilizes office machines to include a computer, fax, and photocopier </li><li>Collects and distributes mail in a timely manner, and in accordance with established clinic procedures </li><li>Participates in huddles </li><li>Schedules appointments </li><li>Takes messages for the Care Team </li><li>Follows all policies and procedures </li><li>Sends authorizations and referrals, completes and documents follow-up on authorizations and referrals to outside agencies, as determined by medical staff to ensure continuity of patient care </li></ul> <p>SPECIFIC JOB COMPETENCIES: (Referrals)</p> <ul> <li>Follows-up on failed appointments via telephone and correspondence for children with special needs. </li><li>Serves as liaison between clinic, school nurses, regional center workers, public health department and other agencies contributing to the care of clinic patients. </li><li>Works with several subspecialist physician offices to prepare paperwork and arrange for scheduling of surgeries and pre-op physicals to maximize the provision of care delivered. </li><li>Assists health department staff with follow-up on CHDP PM-160 Referrals. </li><li>Assist medical staff in the completion of patient forms (ie. DME's, IHSS Program, DMV, Medi-Cal Exemption Forms, Etc.) </li><li>Application for Medical Alert Bracelets. </li><li>Assists patients of the clinic to call their HMO plans to switch their health plan and/or PCP. </li><li>Assists medical staff as an interpreter and cultural facilitator for chronically ill clinic patients </li></ul> <p>EDUCATION AND QUALIFICATION REQUIREMENTS:</p> <ul> <li>Medical Assistant Certification is a plus </li><li>High School Diploma or GED required </li><li>Must have 2-4 years of medical office experience in an out-patient clinical setting. </li><li>Must have demonstrated skill in filling, use of copier equipment. </li><li>Excellent organization and communication skills with attention to detail skills is required. </li><li>Bilingual in Spanish OR Khmer (strongly preferred) </li></ul> <p>Work Environment</p> <p>The noise level of the work environment is usually moderate. Workspace is in an indoor office setting, shared workspace, air-conditioned in the summer and central heat during the winter.
