<p>Pay $35.00 - $40.00 an hour and eligible for shift differentials</p> <p>The Licensed Practical Nurse (LPN) provides direct in-scope nursing care within a 24/7 Supportive Crisis Stabilization Center, delivering timely and compassionate services to individuals experiencing emotional or behavioral health crises.</p> <p>The LPN collaborates closely with registered nurses, behavioral health professionals, peers, and care coordinators to promote client stabilization, safety, and referral to appropriate services in accordance with program regulations.</p> <p>Core Responsibilities</p> <ul> <li>Support clinical triage and assist with observation and documentation of vital signs, symptoms, and responses to interventions. </li><li>Provide trauma-informed and recovery-oriented care in a calm, nonjudgmental, and supportive manner. </li><li>Assist in managing medication-related needs, including administration, monitoring, and coordination with pharmacy providers. </li><li>Collaborate with the multidisciplinary team to assess clients' medical needs and support stabilization and transition planning. </li><li>Schedule appointments and referrals as necessary. </li><li>Participate in the development and monitoring of person-centered safety and care plans. </li><li>Collect and input health information into electronic records; ensure timely and accurate documentation. </li><li>Monitor client health and behavioral changes and alert supervising staff as needed. </li><li>Support individuals with linkage and warm handoffs to ongoing medical, mental health, or substance use services. </li><li>Participate in daily team meetings and huddles and contribute to a collaborative care approach. </li><li>Assist in maintaining a safe and clean clinical environment in accordance with infection control procedures. </li><li>Engage respectfully with individuals in crisis while upholding their rights, dignity, and choices. </li><li>Assist in medical care coordination processes utilizing all relevant third-party resources (HCS, ISTOP, Psyckes etc.) </li><li>Provide daily nursing care to patients in the behavioral health care clinic. </li><li>Assist in accessing RHIO for necessary health information, and lab reports </li><li>Complete all required documentation and reports according to program regulations including program participants' charts. </li><li>Certain deadlines and unanticipated developments may require work during evenings, weekends or holidays to include on-call duties. </li></ul> <p>Qualifications</p> <ul> <li>A.S. Degree or High School Degree or equivalent plus Specialized Trade Certificate. </li><li>Current NY State LPN license and registration. </li><li>1-3 years' experience. </li><li>Experience working with the behavioral health population preferred. </li><li>Familiarity with trauma-informed care, de-escalation practices, and crisis response preferred. </li><li>Ability to work in a fast-paced environment with individuals experiencing emotional distress or acute symptoms. </li><li>Valid NYS Drivers License. </li></ul> <p>Benefits</p> <p>Our work environment provides the opportunity to grow and learn in a fast-paced atmosphere.
