<p>Job Summary</p> <p>This is a Grant funded position.</p> <p>Works in conjunction with the Frederick Health Palliative Care Medical Team and Case Management team to provide skilled professional triage of clinical situations, concerns, and questions, while also delivering skilled professional nursing care to patients as prescribed by the physician and in compliance with state regulations and departmental policies. The RN participates in the interdisciplinary care team to ensure continuity of care for the patient and their family. As part of the Pain and Supportive Care team, they perform clinical support duties for patients across various age groups. This position supports the Frederick Health mission, vision, core values, and customer service philosophy, adhering to the Frederick Health Compliance Program, including regulatory requirements and Standards of Behavior.</p> <p>Essential Functions:</p> <p>Patient and Family Education and Decision Support:</p> <p>Provides comprehensive education and support to patients and their families regarding hospice philosophy, disease progression, treatment options, symptom management, and end-of-life care.</p> <p>Educates and empowers patients and families with clear and accurate information about available end-of-life treatment options, enabling them to make informed decisions aligned with their values and preferences.</p> <p>Clarifies and explains complex medical information, ensuring patients and families understand their condition and care plan.</p> <p>Educates caregivers on how to provide care for the patient in the home environment.</p> <p>Offers guidance and facilitates referrals to assist patients and families in coping with diagnosis and potential outcomes, including advance directives and grief support resources.</p> <p>Acts as a primary resource for patients and families, providing timely support and addressing questions and concerns related to end-of-life care.</p> <p>Keeps appropriate team members apprised of changes and updates to patient/family/caregiver status.</p> <p>Promotes optimum patient comfort as part of an interdisciplinary team under the attending physician's orders.</p> <p>Initiates and sustains implementation of orders as prescribed by the physician.</p> <p>Provides medication, diet, and other instructions as ordered by the physician.</p> <p>Notifies attending physician and Case Managers of any changes in care plan.</p> <p>Is compliant at all times with policies regarding documentation.</p> <p>Monitors documentation in the clinical record and initiates the discharge planning process for assigned patients.</p> <p>Works in concert with the other interdisciplinary team members, assuring that all team members have completed the discharge summary.</p> <p>Works in conjunction with the Frederick Health Palliative Care Medical Team and Case Management team.</p> <p>Provides skilled professional triage of clinical situations, concerns, or questions. Provides skilled professional nursing care to the patient as prescribed by the physician and in compliance with regulations as established by the State Board of Nursing and department policies. The RN participates in the interdisciplinary care team for the patient and family to assure continuity of care. As part of the overall Pain and Supportive Care team performs clinical support duties. Services may be provided to neonate, infant, child, adolescent, adult and geriatric populations.</p> <p>This position supports the Frederick Health mission, vision, core values and customer service philosophy and adheres to the Frederick Health Compliance Program, including following all regulatory requirements and the Frederick Health Standards of Behavior.</p> <p>Required Knowledge, Skills and Abilities:</p> <ul> <li>Demonstrates clinical competency in area of practice. Understands Hospice philosophy, principles of death and dying. Knowledgeable about Hospice eligibility requirements. </li><li>Has excellent observation, assessment, organizational and communication skills, and good nursing judgment. Ability to provide good written documentation in a timely manner. </li><li>Self-directed with the ability to work with little supervision. </li><li>Flexible and cooperative in fulfilling role obligation and has ability to work effectively within an interdisciplinary team. </li><li>Demonstrates ability to assess and respond to the needs of patients and families. Interacts in a calm, courteous and professional manner. </li></ul> <p>Minimum Education, Training, and Experience Required:</p> <ul> <li>Bachelor's degree in Nursing. Associate Degree considered with a BSN commitment. </li><li>Currently licensed as a Registered Nurse in the State of Maryland or Compact State. </li><li>Possesses and maintains current CPR certification. </li><li>At least 2 years general nursing experience is required with current medical, surgical, critical care. Experience providing pain management, Hospice and IV skills preferred. </li></ul> <p>Schedule: 72 hours per bi-weekly pay period.</p> <p>8am-8:30pm 3 days per week. Will work in the Frederick Health Emergency Department</p> <p>Occasional weekends or holidays required.</p> <p>Caring for you as you care for the CommUNITY</p> <p>Frederick Health offers a comprehensive and affordable benefits package. If position is part-time, salary will be pro-rated based on scheduled hours. The pay range may also vary within the stated range based on specialty if applicable. Non-Exempt positions may have shift differential and/or Overtime paid, if applicable.</p> <p>Pay range: $32.00 -$50.00</p>
