<p>The primary objective is to establish rapport, build trust, and facilitate positive change within the community. Performs duties as required to support the elimination of community-based violence.</p> <p> </p> <ul> <li>Develop a comprehensive network of resources that addresses the needs of the population who are victims or perpetrators of community-based violence. </li><li>Develops pre- and post-test questionnaires and other instruments to measure the effectiveness of case management services. </li><li>Prepares analytical and interpretative reports on case management. </li><li>Manage a caseload of residents who are referred to MPHD for Behavioral Health and Wellness </li><li>Coordinate and evaluate services that identify approaches to eliminate gaps in services and resources. </li><li>Review and integrate instructional methods, materials, in in-service provision to 1manage a diverse caseload of participants. </li><li>Participate in the dissemination of program information through attending conferences, seminars, and meetings to promote, publicize, and gain support for the programs. </li><li>Keeps abreast of current developments and trends in the provision of case management services. </li><li>Explore and/or place clients in specialized programs as needed. </li></ul> <p>Any combination of education and experience that would prepare the incumbent to perform the duties of the position at the appropriate level.  Employees would typically have a bachelor's degree and some experience in program management or development.</p> <ul> <li>Certified Case Management (CCM) designation </li><li>Bachelor's degree in social work, psychology, and/or nursing.  </li><li>Knowledge of the principles, methods, and techniques of one or more behavioral, social, or related sciences. </li><li>Experience working with the public. </li><li>Bilingual speaking is a plus </li><li>Ability to conduct interviews, assess needs, and develop an appropriate course of action. </li><li>Ability to research and analyze complex data and write reports. </li><li>Ability to communicate effectively, both orally and in writing. </li><li>Ability to establish and maintain effective working relationships internally and externally. </li></ul>
