Demonstrates VHS Values behavior in all interactions.</p> <p>Following the plan of care assigned by the RN, assesses, identifies problems, implements, evaluates outcomes and documents the care provided.</p> <p>Provides, evaluates and documents patient teaching based on diagnosis and teaching plan. Consults with the RN if the patient/family exhibits a lack of understanding.</p> <p>Documentation is timely, reflects care given and meets policy and regulatory requirements including Core Measure documentation.</p> <p>Participates in educational activities to acquire knowledge and skills appropriate to practice area.</p> <p>Contributes to a supportive work environment. (TJC Standards)</p> <p>Contributes to a safe and healthy work environment (TJC Standards)</p> <p>Provides safe patient care by adhering to the NPSG.</p> <p>Certification & Licensure</p> <p>LPN Nursing License required. Based on primary state of residency and in accordance with current Virginia Board of Nursing Regulations, must be licensed or eligible to practice pending licensure as a Licensed Practical Nurse in the Commonwealth of Virginia with either a: Multi-State License, under the Nurse Licensure Compact OR Single-State License, Valid in Virginia Only.</p> <p>BLS Certification (Basic Life Support) - American Heart 'Healthcare Provider' (HCP) - AHA approved required.
