Coordinates multidisciplinary treatment plans with care team Develops and implements individualized medication plans for transplant patients. Provides medication counseling and education to new transplant patients. Assesses patient health status related to medications and side effects, reviewing all medications, diagnostic tests, and laboratory values. Conducts medication reconciliation and ensures accurate medication histories. Evaluates processes and outcomes of interventions to enhance delivery of cost-effective health care. Coordinates and participates in drug studies related to immunosuppressive therapy and patient compliance. Collaborates with patients, social workers, and financial managers to determine medication selection based on insurance and financial constraints. Educates healthcare personnel on new medications and regimens.
