<p>Description</p> <p>The Aftercare Case Manager will provide in-home case management to individuals who are transitioning out of homelessness and into permanent housing, particularly those who are at risk for or struggling with opioid use disorder. This position is responsible for offering direct support, follow-up care, and coordination of services to ensure long-term housing stability and recovery support. Travel to clients' homes is required to ensure high-touch, personalized care. This is a one-year, grant-funded position, with the possibility of extension based on continued funding and performance.</p> <p>This position requires regular travel to client homes and community locations. Must be able to sit, stand, walk, and lift up to 25 pounds occasionally. Some evening or weekend work may be required based on client needs</p> <p>Work is performed in office settings, residential environments, and in the community. Must be adaptable to changing needs, capable of working in unpredictable settings, and comfortable providing services in client homes.</p> <p>Essential Job Functions</p> <ul> <li>Provide in-home case management services to formerly homeless individuals with a focus on long-term housing retention and substance use recovery; </li><li>Coordinate access to comprehensive wraparound services, including detox programs, peer support, employment resources, and benefits enrollment; </li><li>Conduct periodic home visits and maintain ongoing communication through phone calls, text messages, and in-person check-ins; </li><li>Plan and coordinate client-centered activities and arrange transportation as needed; </li><li>Supply clients with practical resources such as hygiene items, food, and referrals to health and mental health services; </li><li>Work collaboratively with landlords, community providers, and treatment professionals to support housing stability and recovery goals; </li><li>Document all client interactions, progress notes, and service plans in a timely and accurate manner; </li><li>Participate in case conferencing and multidisciplinary team meetings; </li><li>Assist clients in developing independent living skills and maintaining compliance with lease agreements; </li><li>Respond to housing crises and assist with problem-solving to prevent eviction or relapse; and </li><li>All other duties as assigned. </li></ul> <p>Qualifications</p> <p>MINIMUM REQUIREMENTS TO PERFORM WORK:</p> <ul> <li>Associate degree in Social Work, Human Services, or related field (Bachelor's preferred); </li><li>One (1) to two (2) years of experience providing case management or direct services, preferably in housing, substance use, or homeless services; </li><li>Valid driver's license and reliable transportation required; </li><li>Ability to pass background check and drug screening. </li></ul> <p>Knowledge, Skills, and Abilities</p> <ul> <li>Knowledge of housing stability practices, substance use disorders, and community-based recovery models. </li><li>Strong interpersonal and communication skills with the ability to work independently and as part of a team. </li><li>Ability to manage time efficiently, prioritize multiple tasks, and maintain detailed records. </li><li>Sensitivity to the needs of diverse populations and a trauma-informed approach to service delivery. </li><li>Proficiency in Microsoft Office Suite and comfort with digital case management platforms. </li><li>Familiarity with local housing resources, treatment options, and harm reduction strategies is a plus. </li></ul> <p>The City of Columbia is proud to offer a complete benefits package to full-time employees.
