Residents are able to seamlessly transition from independent living to assisted living, and skilled nursing care, as their healthcare needs change.</p> <p>Position Overview</p> <p>The Wellness Nurse plays a pivotal role in supporting the health, wellness, and independence of our independent living residents. This role emphasizes relationship-centered care, proactive wellness strategies, and effective coordination with residents, families, and the care team to achieve the best possible outcomes.</p> <p>Key Responsibilities</p> <ul> <li>Conduct comprehensive wellness assessments to promote holistic well-being. </li><li>Coordinate services and interventions to help residents maintain their independence and enhance quality of life. </li><li>Lead and participate in collaborative care meetings to promote communication and integrated care planning. </li><li>Identify residents at risk and implement targeted strategies to mitigate those risks. </li><li>Serve as a trusted resource and advocate, educating residents and families on health conditions, disease management, and wellness promotion. </li><li>Develop, coordinate, and deliver wellness-focused programs, including health education and disease prevention initiatives </li></ul> <p>Qualifications</p> <p>Education / Licensure:</p> <ul> <li>Active RN or LPN license in FL (required) </li></ul> <p>Experience:</p> <ul> <li>2-3 years of experience in long-term care, post-acute care, or a similar healthcare setting (preferred) </li></ul> <p>Skills and Attributes:</p> <ul> <li>Strong clinical assessment and critical thinking abilities </li><li>Excellent verbal and written communication skills </li><li>A compassionate, resident-centered approach and the ability to build trust and rapport </li><li>Effective organizational and time management skills </li></ul> <p>Why Join Us?</p> <p>At Lake Port Square, we believe in creating a vibrant, engaging, and supportive environment-not only for our residents, but also for our team members.
