<p>Job Summary and Responsibilities</p> <p>As our Wound Ostomy Nurse, you will connect our hospital directly to the community, delivering specialized in-home care that empowers patients to heal comfortably and effectively.</p> <p>Every day you will perform advanced treatments like PICC line care, wound vacs, and infusions, while educating patients and families on complex conditions and treatments.</p> <p>To be successful in this role you must be an exceptionally organized, independent professional, adept at managing complex schedules, patient care, and technology-driven documentation. Your deep commitment to patient engagement and advocacy will make you a vital defense against declining health, fostering a profound impact on individual well-being.</p> <ul> <li>Conducts care assessments and interprets assessment data to determine care needs and constraints in meeting these needs. </li><li>Successfully completes system-based and hospital specific competencies. </li><li>Maintains accurate documentation of observations, treatments, teaching, medications, care given, patient response, and integrates into plan of care. </li><li>In collaboration with patients and families (as defined by the patient) the nurse is responsible for evaluating, planning, directing and providing wound/ostomy care to the patient. Implements a plan of care based upon established standards of care/practice, policies, and procedures, incorporating cultural, religious, psychosocial, physical, and age-related issues. </li><li>Possesses knowledge of medications, and administers safely and accurately. </li><li>Appropriately performs, delegates, and supervises routine treatments to provide wound/ostomy care services. </li></ul> <p>Job Requirements</p> <p>Required:</p> <ul> <li>BSN and Minimum two years of experience as an RN in an acute care facility. , upon hire </li><li>Registered Nurse: KY, upon hire and </li><li>Basic Life Support - CPR, upon hire and </li><li>Certified Ostomy Care Nurse, within 24 - months or </li><li>Certified Wound Care Nurse, within 24 - months or </li><li>Certified Wound Ostomy Continence Nurse, within 24 - months </li></ul> <p>Where You'll Work</p> <p>Welcome to Saint Joseph London, a 150-bed regional hospital founded in 1926.
