<p>POSITION OVERVIEW</p> <p>A Care Manager provides direct care to individuals served with behavioral health needs, including supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative triage, assessment, planning, and communication process. The position requires traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.</p> <p>We are hiring for the following shifts: 8am-4:30pm, 4pm-12:30am, 12am-8:30am</p> <p>JOB BENEFITS</p> <ul> <li>Full Health, Dental, Vision, Disability, Accidental, and 401(k) within 1 month of hire for full-time workers. </li><li>Paid Training and Coaching </li><li>Non-Profit Organization Student Loan Forgiveness </li><li>Wellness Events </li><li>PTO accrual starts on day 1 of employment (full-time staff) </li><li>Company Discount Perks </li></ul> <p>JOB REQUIREMENTS:</p> <ul> <li>Must have a Bachelor's Degree in Human Services or a related field of study toward Human Behavior /Science. </li><li>Must be able to lift and carry up to 50lbs of dead weight </li><li>Must have excellent interpersonal skills </li><li>Valid Florida Driver's license, reliable personal transportation, and insurance. </li></ul> <p>POSITION RESPONSIBILITIES</p> <ul> <li>Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements. </li><li>Complete all clinical and administrative documentation based on established timelines as per program requirements. </li><li>Communicate effectively and maintain cooperative relationships with internal and external resources. </li><li>Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements. </li><li>Maintain efficient workflow management and organization as per program requirements. </li><li>Maintain proficient quality of clinical documentation as per program requirements. </li><li>Other duties as assigned or needed as per program requirements. </li></ul> <p>WORK ENVIRONMENT:</p> <ul> <li>Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe supportive environment. </li><li>This is a 60-bed Adult Crisis inpatient served care facility. </li><li>Office setting within the Crisis center </li><li>Must be capable of lifting 50 pounds of dead weight alone. </li></ul>
