Evaluates clinical and interdisciplinary resident care and makes recommendations based on the community mission statement, current regulatory and best-practice standards. Working with nursing leadership and direct-care staff, the MDS Coordinator is responsible for neighborhood development along with policies and procedures that support the MDS/RAI process.</p> <p>Essential Duties and Responsibilities</p> <ul> <li>Maintains and shares a working knowledge of MDS/RAI requirements, regulations, Shalom Park policies, and practice standards; understands and utilizes the RAI Process for person-directed assessment and development of care plan stories </li><li>Evaluates the extent MDS/RAI person-directed care practices exist; participates in neighborhood rounds to identify and address both quality of care and quality of life issues </li><li>Using the MDS/RAI process, problem solves complex medical/psychosocial situations related to resident choice, and risk </li><li>Evaluates and implements nursing policies and procedures that reflect person-directed care, and evidence based practices; contributes to Shalom Park RAI policy development and ensures policies are utilized by nursing staff </li><li>Models, teaches and utilizes effective communication skills such as active listening, giving meaningful feedback, communicating ideas clearly, dealing with emotional behavior, conflict resolution, and understanding the role of diversity in communication. Demonstrates the importance of leadership by example </li><li>Participates in an effective MDS/RAI quality assurance process that meets regulatory requirements to include timeliness and accuracy </li><li>Coordinates quality assurance performance improvement plans to address quality measures and care concerns </li><li>Takes responsibility not only for their own leadership and personal development, but also develops a culture where staff and resident personal growth is demonstrated </li><li>Understands how the RAI process effects payment and coordinates an accurate MDS process </li><li>Using the MDS/RAI process, supports restorative nursing care to identify person-directed goals and interventions </li><li>Oversees the Medicare A/ sub-acute program to verify skilled care based on best practice and regulatory standards is provided and documented </li><li>Coordinates an effective triple-check process to verify correct billing </li><li>Identifies staff educational needs and works with the Director of Nursing, Human Resources, and Nursing Home Administrator, to provide education </li><li>Works closely with the Admissions Coordinator to coordinate a timely, person-directed admissions process with nursing leadership; evaluates the process to verify resident and family preferences and needs are promptly identified and addressed </li><li>Takes call and provides off-hour leadership support as necessary. </li><li>Other duties may be assigned </li></ul> <p>EDUCATION and/or EXPERIENCE</p> <p>Licensed Practical Nurse (LPN) licensed in the State of Colorado in good standing. Resident Assessment Coordinator-Credentialed (RAC-C) within three months of hire. Related experience as an MDS Coordinator with long-term care experience is preferred.</p> <p>CERTIFICATES, LICENSES, REGISTRATIONS</p> <p>Registered Nurse licensed in the State of Colorado in good standing. Resident Assessment Coordinator-Credentialed (RAC-C) is preferred. Current and active Certified Pulmonary Resuscitation (CPR) certification.</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
