Our job is to partner with the patient's team of supporters from outside the hospital. Through this partnership we will complete an evaluation, initiate a plan of treatment, and arrange appropriate services for the patient and support team that will provide scaffolding following discharge.</p> <p>Summary:</p> <p>Responsible for the evaluation and treatment of an assigned caseload of clients, providing a range of psychosocial care. Provides individual, family and group therapy.
