<p>Job Description</p> <p>Assists all phases of the referral, intake and admissions processes to ensure timely and appropriate admissions of rehabilitation outpatients. Coordinates and monitors the department master therapy schedule.</p> <p>Job Responsibility</p> <ul> <li>Facilitates patient's entry by monitoring and tracking required information and admissions criteria, including physician prescriptions, managed care documents, and financial clearance. </li><li>Maintains outpatient rehabilitation master schedule and "waiting list". </li><li>Monitors occupancy rate and fills all schedule vacancies, as needed. </li><li>Assists with Patient Financial Services personnel to ensure timely and appropriate financial clearance for prospective new admissions. </li><li>Tracks and monitors patient attendance. </li><li>Coordinates and directs externally referred patients based on patients needs. </li><li>Provides written and verbal information to prospective patients. </li><li>Collects and organizes referral and admission data. </li><li>Troubleshoots intake admissions process to resolve issues and resolve process. </li><li>Performs related duties as required.
