This is assignment based -team members will be trained to support the care coordination of patients seeking mental health services in our Pediatric Emergency Department -this assignment requires day, evening, and night shift ADCP staffing and alternating weekends. This role also supports the discharge management of hospitalized children transitioning back home, to a facility, or to another hospital for ongoing care-this assignment requires day, early evening and alternating weekend staffing.</p> <p>Associate Discharge Planner On-site Work Requirement:</p> <p>Evening Shift 4pm-12am</p> <ul> <li>Part Time Rotating Weekends (3 shifts a week) </li><li>1 overlapping shift a week float to CM Team Support </li></ul> <p>Experience requirements: 3 years +</p> <p>Licensure required: None</p> <p>.</p> <p>KEY RESPONSIBILITIES</p> <ul> <li>Assists treatment team in locating resources necessary to patients' safe discharge. </li><li>Assists in identifying service gaps and/or system issues that interfere with timely/appropriate discharge. </li><li>Demonstrates ability to advocate for patients and families to referral sources. </li><li>Identifies, maintains, and updates information on agencies and resources necessary for patients' continuing care. </li><li>Maintains accurate records. </li><li>Contributes to the organization by actively participating in process improvement initiatives and shared decision making. </li><li>The responsibilities listed are a general overview of the position and additional duties may be assigned. </li></ul> <p>TECHNICAL CAPABILITIES</p> <ul> <li>Planning & Organizing (Intermediate): Demonstrates ability to anticipate complex resource needs, identify discrete tasks, set priorities, define dependencies, schedule activities, meet deadlines, and organize work beyond areas of direct responsibility. Possesses mastery of the core technology, products, services and operations within field of work and is fully capable of their delivery without requiring support and instruction from others. Able to train and educate by setting the example and giving technical instruction. Demonstrates acute knowledge of industry trends and emerging technologies. Attends industry seminars and forums on a regular basis. </li><li>Family Advocacy (Intermediate): Demonstrates mastery of family advocacy in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job. </li><li>Patient Documentation (Novice): Possesses sufficient fundamental proficiency to successfully demonstrate patient documentation in practical applications of moderate difficulty.
