<p>Title: Centering Pregnancy Coordinator</p> <p>Reports to: Director of Quality Improvement (DQI)</p> <p>FLSA Status: Non-Exempt Personnel Supervised: None</p> <p>POSITION SUMMARY:</p> <p>The Centering Pregnancy Coordinator is a minimum of a four-year grant funded position, with the opportunity of ongoing employment through a successful program. The Centering Pregnancy Coordinator is a key participant of the Steering Committee, coordinating activities necessary to achieve and maintain quality and sustainability of Centering. This individual provides administrative support to Centering Pregnancy and serves as a liaison to Centering Healthcare Institute as needed to support the Centering Pregnancy practice. The Centering Pregnancy Coordinator is expected to work well with every member of the clinical team to provide safe, high quality and efficient care to a patient population diverse in age, health status, and socio-economic level. This position requires strong teamwork and communication skills with the care team.</p> <p>MINIMAL QUALIFICATIONS:</p> <p>Graduate of an accredited Medical Assistant Program and Certified or Registered by a Medical Assistant Accrediting Organization and/or</p> <p>Licensed Practical Nurse with current Florida License</p> <p>Current CPR Certification</p> <p>Minimum of 2 years of experience in the medical field required, preferably maternal health</p> <p>Exceptional team, communication and written/oral skills</p> <p>Computer literacy, Proficient in using Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, Teams)</p> <p>Knowledge of medical terminology and electronic health records</p> <p>Ability to work under tight deadlines and remain flexible</p> <p>Ability to maintain confidentiality and handle sensitive information with discretion</p> <p>Travel between sites requiring reliable transportation</p> <p>Bilingual Preferred</p> <p>RESPONSIBILTIES AND PERFORMANCE EXPECTATIONS include, but are not limited to, the following:</p> <ol> <li> <p>Collaborate with Grant Coordinator for all reporting responsibilities and data collection pertaining to the Florida Blue Foundation Grant</p> </li><li> <p>Centering Steering Committee Member and Coordinator</p> </li></ol> <p>a. Key member of the Steering Committee</p> <p>b. Participates in developing agendas for Steering Committee meetings</p> <p>c. Facilitates and/or takes minutes at meetings</p> <p>d. Follows up with individuals on action items</p> <p>e. Communicates with staff about the work of the Steering Committee</p> <ol start="3"> <li>Centering Participation, Evaluation, and Administrative Support </li></ol> <p>a. May co-facilitate Centering groups</p> <p>b. Ensures master schedules are developed and maintained for Centering groups (includes coordination of dates, space, schedules of the Centering provider facilitators and staff facilitators)</p> <p>c. Manages logistical issues for Centering groups including:</p> <p>i. ordering supplies and materials</p> <p>ii. snacks</p> <p>iii. assessment equipment and supplies</p> <p>iv. room set up and break down</p> <p>d. Enrolls Centering participants and/or monitors the enrollment process to ensure optimal group size; orients staff who will be involved in the enrollment process</p> <p>e. Arranges for new hire staff and providers to receive Centering facilitation training</p> <p>f. Serves as liaison with Centering Healthcare Institute and/or Centering Implementation Advisor for startup, ongoing maintenance and problem solving, as needed</p> <p>g. Collects and reviews patient satisfaction surveys and facilitator process evaluations</p> <p>h. Oversees and/or conducts the ongoing data collection and reporting</p> <p>i. Prepares for initial site accreditation visit and submits data for continued accreditation status</p> <p>j. Trains and acts as point person for volunteers or students assisting with Centering, if applicable</p> <p>k. Provides outreach to other departments and community stakeholders and agencies to educate others about Centering</p> <ol start="4"> <li> <p>Screen patients according to CFHC's protocols (obtaining height, weight, vital signs)</p> </li><li> <p>Accurately obtain patient's medical history.</p> </li><li> <p>Educate patients per treatment plan (diet, exercise, medication)</p> </li><li> <p>Prepare and administer injections according to position requirements (subcutaneous, intramuscular, and intradermal) per provider orders</p> </li><li> <p>Document procedures and patient responses in the patients' electronic medical record (EMR)</p> </li><li> <p>Maintain strict infection control (maintaining, performing procedures, needle disposal)</p> </li><li> <p>Understand the requirements of CFHC's Exposure Control Plan and utilize universal precautions</p> </li><li> <p>Maintain exam rooms and work areas (keeping rooms well-stocked, orderly, and clean, to include disinfecting exam tables between patients as per infection control policy)</p> </li><li> <p>Accurately make/cancel/reschedule appointments in the computer appointment system</p> </li><li> <p>Complete and ensure accuracy of clinical logs (Centering documents/reports, medication administration)</p> </li><li> <p>Maintain patient concerns within EMR, communicate with patients via telephone, patient portal, keep team members updated regarding clinical changes</p> </li><li> <p>Comply with all policies, procedures, state, and federal medical regulations</p> </li><li> <p>Report as needed to Clinical Services Manager (CSM) or DQI any pertinent information impacting patient care</p> </li><li> <p>Attend and participate in mandatory meetings (center Specific meetings, Corporate meetings, and other meetings as designated by the CSM, DQI or CMO)</p> </li><li> <p>Excellent communication and interpersonal skills</p> </li><li> <p>Collaborate with physicians and other members of the clinical team</p> </li><li> <p>Complete all duties, responsibilities, and competencies in a manner that is appropriate to the age groups to whom care/services are being provided.</p> </li><li> <p>Maintain a neat and professional appearance at all times</p> </li><li> <p>Other duties as assigned by the clinician, CSM, DQI, or CMO.</p> </li></ol> <p>PHYSICAL REQUIREMENTS:</p> <p>(Subject to Change)</p> <ol> <li> <p>Requires 80% or more time spent standing/walking</p> </li><li> <p>Independently mobile</p> </li><li> <p>Ability to lift weight equivalents to what would be required when occasionally assisting and positioning patients, repositioning equipment, and lifting supplies</p> </li><li> <p>Ability to adapt and function in varying environments of workload, patient acuity, worksites, and work shifts.</p> </li></ol> <p>American with Disabilities Act (ADA) Statement: External and internal applicants, as well as position incumbents who become disabled, must be able to perform the essential job specific functions (listed within each job responsibility) either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case-by-case basis.</p>
