<p>The Registered Nurse (RN) PACT Coordinator is responsible for providing competent, evidence-based practices. This nurse has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. Executes position to promote full PACT implementation and function within the Primary Care setting.</p> <p>The Registered Nurse (RN) PACT Coordinator delivers fundamental knowledge-based care to assigned clients while developing technical skills. Serves in an administrative role as a Primary Care Patient Aligned Care Team (PACT) champion and coordinates local PACT implementation, training, communication, and sustainability of the PACT model. Actively promotes the PACT model within the Ambulatory Care setting, educates core PACT team members, identifies, and addresses communication and workflow challenges, collaborates with leadership and associated stakeholders, and schedules meetings among teams.</p> <p>Duties include but not limited to:</p> <ul> <li>Uses knowledge of PACT principles to support PACT team care delivery. </li><li>Assists core PACT teams in organizing care delivery. </li><li>Monitors fidelity to the PACT model of care. Offers training and education relating to the PACT model of care </li><li>Completes orientation according to expected standards. </li><li>Works with close supervision, is responsible and accountable for individual nursing practice and seeks direction from others as needed. </li><li>Manages workload as assigned, organizes, and completes own assignments in an efficient and appropriate manner. </li><li>Participates in the development, implementation, and evaluation of interdisciplinary care. </li><li>Assists with initiatives to improve systems and processes within the clinic setting. </li><li>Assists staff in elevating ethical issues that impact programs, processes, and organizational practices. Follows established policies of the practice setting, VA, and ANA Code of Ethics for Nurses. </li><li>Models high reliability principles (HRO) and ICARE values to improve the consistency of care delivery and patient outcomes. </li><li>Promotes processes for PACT model care, which is focused on Whole Health patientdriven care, education, self-management, and customer satisfaction. </li><li>Integrates patient preferences into care delivery processes, sharing knowledge with peers within the immediate practice setting. </li><li>Utilizes data to improve the patient experience. This may include data from Survey of Healthcare Experience of Patients (SHEP), Veteran Signals (VSignals), Strategic Analytics for Improvement and Learning (SAIL), Patient Advocate Tracking (PATS), All Employee Survey (AES), and any other data resources that may be utilized to analyze the patient experience. </li><li>Maintains professional relationships within the facility to enhance the Veteran's experience. </li><li>Other duties as assigned. </li></ul> <p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards</p> <p>Pay: Pay: Competitive salary, regular salary increases, potential for performance awards</p> <p>Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Licensure: 1 full and unrestricted license from any US State or territory</p> <p>Work Schedule: 8:00am - 4:30pm, Monday - Friday, (Work schedule is subject to change to accommodate the needs of the agency).</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Relocation/Recruitment Incentives : May be Authorized</p> <p>Permanent Change of Station (PCS): Not Authorized</p>
