If you are passionate about healthcare and providing excellent patient care, we want to hear from you!</p> <p>Position: Patient Care Coordinator</p> <p>Location: Layton</p> <p>Type: Part-Time</p> <p>Schedule: Saturday & Sunday 9 am - 3 pm; Evening Shifts as Needed (PRN)</p> <p>Job Description:</p> <p>As a Care Coordinator at Wee Care Pediatrics, you play a crucial role in ensuring the seamless and coordinated care of our young patients. Your responsibilities include collaborating with medical professionals, facilitating referrals, managing prescription fulfillment, overseeing lab results, serving as a patient point of contact, and ensuring the continuity and quality of care.</p> <p>Job Duties:</p> <ul> <li>Provide patients with appropriate referrals to specialists and additional healthcare services. </li><li>Manage prescription fulfillment and refills, ensuring timely access and accurate records. </li><li>Receive, document, and communicate lab results for timely follow-up and care decisions. </li><li>Act as a patient advocate, addressing questions and providing support and information. </li><li>Coordinate patient care transfers between departments or facilities for continuity of care. </li></ul> <p>Job Qualifications</p> <ul> <li>Previous MA work experience desired; experience in Pediatrics preferred. </li><li>Understand various lab tests and able to clearly communicate results and provider instructions to parents. </li><li>Answer basic medical questions and offer advice to parents over the phone ensuring high standard of care. </li><li>Ability to communicate effectively with providers, staff, and patients/parents. </li><li>Capable of prioritizing multiple job demands and work in a fast-paced health care environment. </li><li>Demonstrate compassion and understanding for the patient and families. </li><li>Basic computer knowledge. </li></ul>
