<p>Position Summary</p> <p>The Community Engagement Program Manager (CEPM) coordinates care for referrals to the Home Visiting Department (HVD) to ensure families are linked to the home visiting program that best matches their characteristics and are that they are served in a timely manner. This position will be responsible for developing and maintaining ongoing partnerships with community and area partners to raise awareness of HVD. This position also acts as the liaison to the HVD internally within Mary's Center. The CEPM is responsible for facilitating and coordinating a robust community engagement strategy for the HVD, with a particular focus on building relationships with local community partners and service providers and coordinating the Community Advisory Board (CAB) for the HVD. This position works closely and provides support to the Home Visiting Management team to ensure caseload capacities are met according to implementing model and grant requirements of the different teams within the HVD.</p> <p>Reportability</p> <p>This position will report directly to the Home Visiting Director.</p> <p>Essential Duties and Responsibilities</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.</p> <p>Intake and Community Engagement</p> <ul> <li>Receives all internal and external referrals for the HVD via phone, email, website, and fax, and triages each referral to the appropriate team inside the HVD within two business days to ensure families are linked to the program that best matches their characteristics in a timely manner. </li><li>Contacts referral source and/or family to obtain additional information as required to complete program assignment. </li><li>Initiates data entry of new referrals and documents participant engagement efforts on electronic health record and other data bases as required by grant funders. </li><li>Keeps track of referrals and their outcomes. Reviews referral demographics and characteristics to assess how program is meeting community needs. </li><li>Prepares reports about community outreach as needed, drawing conclusions about program effectiveness identifying potential reasons for the results. </li><li>Works with leaders to create and implement HVD outreach strategy to ensure programs within the HVD meet capacity based on model and grant requirements. This includes the development of quality improvement projects that identify and prioritize program goals and activities to be complete. </li><li>Works with leaders to develop appropriate performance indicators needed to assess CQI progress and outcomes related to outreach and program capacity. </li><li>Serves as the HVD liaison to the C3 initiative, and other strategies that support care coordination within Mary's Center. </li><li>Cultivates relationships with and represents Mary's Center in assorted city-wide coalitions and alliances. This may entail significant local travel between and among the jurisdictions that encompass the Washington, DC metropolitan area. Develops MOUs as appropriate and identified. </li><li>Builds and maintains strong relationships with key stakeholders, including governmental, non-profit, faith leaders, private, schools and academics. </li><li>Facilitates regular community meetings to foster relationship building, collaboration, and promotes equitable services for families with children 0-5 years old. This includes strengthening relationships with partners, identifying and addressing improvements to working together to authentically support families of young children including implementation of equitable and best practices and continued communication strategies. </li><li>Coordinates participation of HVD in outreach events and keeps outreach calendar up to date. </li><li>Recruits, trains, and supervises volunteers and supports staff to assist with special events and other activities aligning with the HVD's opportunities for volunteer engagement. </li><li>In conjunction with the MarCom and program staff, develops content for and helps implement an external communications plan that engages our communities in our work to include but not limited to: </li><li>Provides ongoing input, information, and materials (as requested) that supports internal and external communication efforts about programming and opportunities, </li><li>Develops flyers, one pagers and other program materials, </li><li>Creates content for regular community and business newsletters, </li><li>Timely updates to HVD page on Mary's Center website, and </li><li>Regular outreach to local and regional audiences through other organizations and publications, </li><li>Seeks opportunities for HVD and Mary's Center to be a partner or sponsor of other organizations' events, particularly those which serve the HVD target population. </li></ul> <p>Volunteers</p> <ul> <li>In partnership with HV Director and program leadership, recruits, develops, and maintains a diverse and inclusive volunteer corps for both support of program operations and Community Advisory Board (CAB), collecting information on qualifications and skills and arranging for training when needed. </li><li>Works with the Home Visiting Director and program leadership to assign responsibilities and communicates as appropriate, ensuring volunteers are satisfied by creating a collaborative and fun working environment. </li><li>Works with the Home Visiting Director to track information and engagement regarding the CAB. </li><li>Builds relationships with volunteers and networks who work in areas related to HVD's mission and initiative. </li></ul> <p>Management</p> <ul> <li>Attends and participates in scheduled seminars, conferences, and in-service training to build relationships with community and identify and address needs. </li><li>Coordinates and manages the CAB for the HVD. Works with the Home Visiting Director and program leadership to develop agenda items and prepare presentations for the CAB as needed. </li><li>Assists Home Visiting Director in developing program budget and tracking expenses related to program outreach and recruitment. </li><li>Other activities as identified by the Home Visiting Director. </li></ul> <p>Qualifications</p> <p>The requirements listed below are representative of the knowledge, skills, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p> <p>Knowledge, Skills & Abilities -</p> <ul> <li>Bachelor's degree in human services or fields related to working with community outreach with 3 years of relevant experience. </li><li>A solid understanding of and experiencing building partnerships. </li><li>Administrative experience in human services or related field, including experience in quality assurance/improvement. </li><li>A working knowledge and understanding of patient/client confidentiality issues and HIPAA compliance/regulations are essential. </li></ul> <p>Language Requirement - Ability to communicate effectively (written and oral) in English is required. Additional language proficiency or fluency preferred.</p> <p>Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.</p> <p>Work Environment - Mostly in a typical office and/or home setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
