Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services.</p> <p>Our community Health Care Centers are located at 281 Main St. East Hartford, 40 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity's Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area.</p> <p>We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.</p> <p>AMAZING Benefits!</p> <p>At Intercommunity, we believe our benefits should make a difference to you, to your family and to the life you lead outside of work. Benefits take effect on the first day of the month following the hire date.</p> <p>Summary:</p> <p>The Care Coordinator at Withdrawal Management is responsible for care coordination for all clients on the unit, including admission and discharge coordination.</p> <p>Essential Duties & Responsibilities:</p> <ul> <li>Works collaboratively with the clinical team and the medical team to support client's aftercare plan. </li><li>Work collaboratively with the admission team to ensure appropriate client admissions. </li><li>Facilitates assigned scheduled groups and documents as required. </li><li>Facilitates phone and in person screenings for aftercare planning. </li><li>Prepares and faxes referrals to aftercare programs. </li><li>Secures transportation for discharges. </li><li>Assists in client registration and appointment scheduling. </li><li>Serves as back up to our insurance authorization process. </li><li>Assists with client care duties on the unit as required. </li><li>Other related duties as assigned. </li></ul> <p>Schedule:</p> <p>9 AM - 5:30 PM - Full-Time, 5 days per week - Must be available to work a minimum of 2 weekend days per month</p> <p>Requirements</p> <p>Education &/Or Experience:</p> <p>Bachelor's Degree, Peer Support training (as identified by Advocacy Unlimited training) preferred, or a minimum 2 years of experience in human services field. CAC certification preferred.</p>
