This is an opportunity to guide a multidisciplinary team, shape innovative care coordination on Chicago's West Side, and directly impact health equity for some of the city's most underserved communities.</p> <p>As the Team Leader, you will provide both clinical and operational leadership for a team including a Behavioral Health Care Manager, Community Health Worker and a Peer Support Specialist within a Community Mental Health Center (CMHC) setting.
