<p>If you are an existing employee of South Shore Health then please apply through the internal career site.</p> <p>Requisition Number:</p> <p>R-22729</p> <p>Facility:</p> <p>LOC0001 - 55 Fogg Road55 Fogg Road Weymouth, MA 02190</p> <p>Department Name:</p> <p>SSH Palliative Care APC PB</p> <p>Status:</p> <p>Part time</p> <p>Budgeted Hours:</p> <p>0</p> <p>Shift:</p> <p>Day (United States of America)</p> <p>Performe consultations and provide assessments for pain/symptom management, prognostication and goal-setting, decision making, and hospice engagement.</p> <p>Compensation Pay Range:</p> <p>$153.00 - $200.00</p> <ul> <li> <p>Physician shall perform the following duties as a Practitioner of the Palliative Care Program:</p> </li><li> <p>Perform consultations, as requested, with assessments for pain/symptom management, prognostication and goal-setting, decision making, and hospice engagement.</p> </li><li> <p>Collaborate with other Hospital departments and associated non-Hospital entities to ensure continuity of supportive and palliative patient care amongst relevant entities.</p> </li><li> <p>Attend periodic continuing medical education in topics related to supportive and palliative care.</p> </li><li> <p>Participate in providing supportive and palliative care education.</p> </li><li> <p>Appropriately document all services and communicate with "primary" physicians as needed.</p> </li><li> <p>Partner with patients and families to involve them in a Patient and Family Centered Health Care System that includes informed decision making, as well as reliably meeting patient's needs and preferences.</p> </li></ul> <p>Education</p> <ul> <li> <p>MD</p> </li><li> <p>Successful completion of an ACGME- or AOA-accredited residency in Anesthesiology, Emergency Medicine, Family Medicine, General/Vascular Surgery, Internal Medicine, Medical Imaging, Neurology, Obstetrics/Gynecology, Pediatrics, Physical Medicine & Rehabilitation, or Psychiatry.</p> </li><li> <p>Current subspecialty certification in Hospice and Palliative Medicine or active participation in the exam process.</p> </li></ul> <p>Minimum Experience</p> <ul> <li> <p>Meets requirements of South Shore Health medical staff bylaws</p> </li><li> <p>Credentialed by SSH medical staff</p> </li></ul> <p>per diem</p> <p>Responsibilities if Required:</p> <p>Education if Required:</p> <p>License/Registration/Certification Requirements:</p>
