Offers may vary depending on the circumstances of each case.</p> <p>Summary:</p> <p>The Affirm Licensed Clinical Social Worker is an integral member of the Affirm program which focuses on services for the LGBTQ community. This role works in a collaborative relationship with the Director of Community Based Practices and the Affirm Program Manager to support patients, families, and community partners. The social worker is an expert in biopsychosocial assessment, mental health issues, care planning and coordination of care across medical and community-based service systems in order to reduce psychosocial barriers that may negatively impact the patient's health management and outcomes. The individual who holds this position understands population management especially as it pertains to the LGBTQ+ communities, exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures, including complying with all Rush University Medical Center Customer Service Standards.</p> <p>Other information:</p> <p>Required Job Qualifications:</p> <ul> <li>Master's degree in Social Work. </li><li>Licensed Clinical Social Worker (LCSW) in State of Illinois. </li><li>4 years' experience in healthcare, care management, mental health and/or social services. </li><li>Clinical experience with relevant population(s). </li><li>Proficiency with Microsoft Office Suite. </li><li>Strong active listening, critical thinking, judgment and decision-making skills. </li><li>Shows acceptance of each person and their life journey. </li><li>Ability to approach clients and workplace from a trauma-informed and health equity lens. </li></ul> <p>Preferred Job Qualifications:</p> <ul> <li>Experience providing clinical supervision. </li><li>Experience with evidenced-based interventions. </li><li>Experience with high risk and/or complex populations. </li><li>Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability. </li><li>Strong planning and implementation skills, including problem solving and troubleshooting skills. </li><li>Excellent verbal and written communication, interpersonal and organizational skills. </li></ul> <p>Physical Demands:</p> <p>Disclaimer: The above is intended to describe the general content of and requirements for the performance of this job. It is not to be construed as an exhaustive statement of duties, responsibilities or requirements.</p> <p>Responsibilities:</p> <ul> <li>Collaborates with the Affirm team to identify patients in need of social work and behavioral health services. </li><li>Performs and documents comprehensive psychosocial assessments. </li><li>Develops and documents evidence-based individualized care for identified patients, focusing on specific psychosocial goals, including assessments and preparation of Letters of Readiness for gender affirming care in accordance with the WPATH Standards of Care. </li><li>Develops evidence-based plans of care that address barriers with the patient and care team member to achieving mental health and function. </li><li>Provides psychosocial education, counseling, anticipatory guidance, and linkage to community resources during both individual and group sessions for patients and families as indicated by comprehensive assessment. </li><li>Advocates within complex medical and community-based service systems to improve patient access to needed services. </li><li>Facilitates communication between patients, care teams and community-based providers to ensure continuity of care. </li><li>Provides educational materials and resources to patient/family that are in a format which is specific to the patient/family learning abilities and language preference. </li><li>Effectively collaborates with inter-professional healthcare teams within and outside of the Rush system. </li><li>Works to improve identified CBP/CHEE outcomes. </li><li>Participates in quality improvement initiatives. Activities may include data collection, chart review, inter-professional collaboration, analysis of patient data, and care team meetings </li><li>Attends individual and group supervision meetings for outpatient social workers as well as relevant social work trainings. </li><li>Participates in the recruitment and supervision of social work interns, as required. </li><li>Participates in weekly team meetings and scheduled huddles with the wider Affirm team. </li><li>Provide support to Patient Navigation team through case conferencing and call escalations, </li><li>Supports Affirm's community outreach initiatives, including group psychotherapy and education, as required. </li><li>Performs related duties as required. </li></ul> <p>Rush is an equal opportunity employer.
