<p>Director of Healthcare Social Impact</p> <p>Department: Equity & Inclusion</p> <p>Shift: 9AM-5PM</p> <p>Job Summary</p> <p>The Director of Healthcare Social Impact (Director) is a mission-driven leader responsible for developing and implementing programs that address the root causes of health disparities and improve community well-being. This role advances Cook County Health's commitment to health equity, social justice, and transformative community partnerships as a safety-net health system supporting John H. Stroger, Jr. Hospital and Provident Hospital. The Director will lead cross-functional strategies that support the hospital's role as an anchor institution and a trusted community partner.</p> <p>Typical Duties</p> <ul> <li>Develops and leads systemwide initiatives focused on social determinants of health (SDOH), including housing, food insecurity, behavioral health, and workforce development. </li><li>Integrates social impact priorities into hospital-wide strategy, aligning with population health, Medicaid transformation, and value-based care efforts. </li><li>Serves as a strategic advisor to executive leadership on community health trends and social impact opportunities. </li><li>Designs, launches, and scales innovative programs that address non-medical drivers of health and reduce disparities for historically marginalized populations. </li><li>Leads cross-departmental projects with clinical teams, care management, behavioral health, and external stakeholders to create integrated care models. </li><li>Identifies opportunities for shared-value partnerships with community-based organizations (CBOs), housing authorities, and public health agencies </li><li>Cultivates long-term partnerships with local nonprofits, advocacy groups, government entities, and public health agencies. </li><li>Represents the hospital in community coalitions, health equity forums, and cross-sector working groups. </li><li>Elevates community voice and lived experience in program design and decision-making. </li><li>Develops and manages systems for measuring social impact, equity outcomes, and program performance. </li><li>Partners with data analytics teams to assess and communicate outcomes such as reduced Emergency Department utilization, improved care continuity, or SDOH screening uptake. </li><li>Prepares reports, dashboards, and presentations for funders, the Board of Trustees, and public stakeholders. </li><li>Identifies funding sources including philanthropic grants, public-private partnerships, and Medicaid waivers. </li><li>Collaborate with grant writers, philanthropy teams, and external funders to secure support and ensure fiscal accountability. </li><li>Builds business cases for investments in social care interventions that improve health and reduce costs. </li><li>Develops systems and processes to determine effectiveness of program activities </li><li>Interfaces effectively with external stakeholders </li><li>Serves as project lead or key staff for cross functional projects </li><li>Works with internal and external partners to develop budgets, create work schedules, and administer several CCHHS housing initiatives and partnerships to include existing or planned medical respite and the flexible housing pool. </li><li>Reports to the Chief Equity & Inclusion Officer. </li><li>Performs other duties as assigned or needed. </li></ul> <p>Minimum Qualifications</p> <ul> <li>Bachelor's degree from an accredited college or university </li><li>Five (5) years of experience in community health, policy advocacy, health equity and/or healthcare social impact programs </li><li>Two (2) years of program management experience </li><li>Two (2) years of experience supervising and/or managing staff </li></ul> <p>Preferred Qualifications</p> <ul> <li>Master's level preparation in social services, public health or public policy </li><li>Experience with those with MH/SUD or co-occurring disorders who are experiencing homelessness </li><li>Experience working with a care coordination entity </li><li>Previous health care experience </li></ul> <p>Knowledge, Skills, Abilities and Other Characteristics</p> <ul> <li>Knowledge of clinical resources and therapeutic options for individuals with Mental Health/Substance Use Disorder (MH/SUD) </li><li>Knowledge of the interplay between poverty, trauma, mental illness, medical challenges, incarceration, substance use disorder, and aging on ability to remain housed </li><li>Knowledge and proficiency using Microsoft office </li><li>Excellent verbal and written communication skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups </li><li>Strong customer service and empathy skills </li><li>Ability to interface effectively with stakeholders representing diverse organizations and constituencies. </li></ul> <p>VETERAN PREFERENCE</p> <p>PLEASE READ</p> <p>When applying for employment with the Cook County Health& Hospitals System, preference is given to honorably discharged Veterans who have served in the Armed Forces of the United States for not less than 6 months of continuous service</p> <p>To take advantage of this preference a Veteran must:</p> <ul> <li>Meet the minimum qualifications for the position. </li><li>Identify self as a Veteran on the employment application by answering yes to the question by answering yes to the question, "Are you a Military Veteran?" </li><li>Attach a copy of their DD 214, DD 215 or NGB 22 (Notice of Separation at time of application filing.
