<p>The Clinician II serves as a Recreation Coordinator which is primarily responsible for providing guidance and leadership to a team of direct support professionals who support and build skills for individuals with disabilities, ensuring a high quality of care for all participants. The Recreation Coordinator is also responsible for coordinating services and support through a person-centered plan that addresses complex behaviors, medical needs, and crisis intervention.</p> <p>And here's the exciting part… A $4,000 hiring bonus is being offered for this position.</p> <p>Specific Duties:</p> <ul> <li> <p>Develop, implement, and monitor individual service plans (ISPs) for each participant, ensuring outcomes and essential supports align with their needs and abilities.</p> </li><li> <p>Manage behavioral and crisis situations utilizing Therapeutic Options Techniques.</p> </li><li> <p>Administer medications without error and utilize the appropriate process outlined in the policy.</p> </li><li> <p>Oversee up to three (3) program activity rooms. Each room consisting of approximately seven (7) individuals, and two (2) support staff</p> </li><li> <p>Develop and implement Individualized Support Plans (ISPs) for assigned individuals, including conducting assessments, participating in planning meetings, and coordinating services interdisciplinary team.</p> </li><li> <p>Upload all licensing required information into the EHR and designated state database system as required.</p> </li><li> <p>Utilize Electronic Health Record (EHR) system to accurately document and manage client information in compliance with organizational and regulatory standards.</p> </li><li> <p>Ensure all licensing documentation is completed and submitted accurately and on time to maintain compliance with division and regulatory requirements.</p> </li><li> <p>Ensure program activities are person centered and align with the individual's goals and needs.</p> </li><li> <p>Uphold all confidentiality, Human Rights, and Home and Community Based Service (HCBS) Rights at all times.</p> </li></ul> <p>The City offers a?generous benefits package, which includes health, dental and life insurance, retirement and savings plans, maternity/paternity and parental leave, holidays and paid leave.</p> <p>Hiring bonuses shall only be for new hires of original full-time employment or reemployment as defined in Human Resources Policy No. 5.0.</p> <ul> <li> <p>$1,000 will be awarded in the first paycheck.</p> </li><li> <p>$1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of "Meets Expectations".</p> </li><li> <p>$1,750 will be awarded upon completion of the employee's one (1) year probationary period and a performance evaluation with an overall rating of "Meets Expectations".</p> </li></ul>
