<p>JOB SUMMARY# The Ambulatory Clinical Pharmacist will serve as an integral member of our Value-Based Care (VBC) team. This role focuses on optimizing medication therapy management (MTM),#improving patient medication adherence, and reducing healthcare costs through#collaboration with their primary care physician and specialist. The ideal candidate will work#closely with the adult and senior population, with the aim to build strong relationships to#resolve any barriers to a patient#s medication regimen. Additionally, the Ambulatory Clinical#Pharmacist will have the responsibility to addressing high-risk medication use, managing high-cost specialty drugs, and reconciling medications post-discharge to mitigate the risk of#readmission.# MINIMUM REQUIREMENTS# # Licensed or eligible to be licensed as a pharmacist in the state of Indiana. PharmD#or Bachelor#s degree in pharmacy required.# # Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory#Care Pharmacist (BCACP) preferred.# # 3-5 years of clinical pharmacy experience in an ambulatory healthcare setting,#especially in primary care, ambulatory quality or other clinical practice#settings, is#preferred. Monitor clinical outcomes and adjust pharmacotherapy to meet quality metrics#and value-based performance targets.# E. Utilize electronic health records (EHR) and population health tools to identify high-risk patients and track interventions.# F. Participate in quality improvement initiatives and contribute to the development of#clinical guidelines and protocols.# G. Provide medication-related consultation to providers and healthcare staff.# H. Stay current with evidence-based guidelines, formulary updates, and emerging#therapies.# I. Assist in coordinating care transitions to reduce hospital readmissions and#improve continuity of care.# J.
