We are excited to continue to grow our mission family!</p> <p>Benefits offered:</p> <p>Medical, Dental & Vision</p> <p>Vacation, Sick & Holiday Paid Time Off</p> <p>Mileage Reimbursement</p> <p>Employee Assistance Program</p> <p>Flexible Spending & Health Savings Accounts</p> <p>Disability, Life and AD&D Insurance</p> <p>401(K)</p> <p>Pay range: $56-$60/hr.</p> <p>Benefits offered:</p> <p>Medical, Dental & Vision</p> <p>Vacation, Sick & Holiday Paid Time Off</p> <p>Mileage Reimbursement</p> <p>Employee Assistance Program</p> <p>Flexible Spending & Health Savings Accounts</p> <p>Disability, Life and AD&D Insurance</p> <p>401(K)</p> <p>Responsibilities:</p> <ul> <li>Assess patients' physical, psychological, social, cultural, and spiritual needs. </li><li>Develop individualized care plans focused on symptom management and quality of life. </li><li>Coordinate care with interdisciplinary team members for comprehensive support. </li><li>Educate patients and families on hospice philosophy and end-of-life care. </li><li>Monitor patient progress, adjusting care plans as needed. </li><li>Facilitate seamless communication to ensure continuity of care. </li><li>Conduct follow-ups to maintain care compliance and resolve concerns. </li><li>Identify and address barriers to successful care transitions. </li><li>Advocate for patient needs from pre-admission through discharge. </li><li>Collaborate with clinical teams to source hospice referrals. </li><li>Review Explanation of Benefits (EOBs) and consent documents with patients. </li><li>Perform nursing assessments to evaluate hospice eligibility. </li><li>Admit qualifying patients for general inpatient hospice care. </li><li>Support hospice service line through sales, marketing, and public relations efforts. </li></ul> <p>Qualifications:</p> <ul> <li>Nursing degree from an accredited school; bachelor's preferred. </li><li>Minimum of 1year of hospice care experience, plus 1+ years in acute and home care settings. </li><li>Active state nursing license and BLS CPR certification. </li><li>Supervises hospice aides; prior management experience not required. </li><li>Strong assessment, communication, problem-solving, and nursing skills. </li><li>Ability to lift, transfer, and position patients; frequent movement required. </li><li>Valid driver's license with insured, reliable transportation. </li><li>Preferred experience in holistic or alternative care modalities. </li></ul> <p>See what Mission has to offer!
