<p>Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on skin and wound etiology. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout patient care. Administers medications and procedures per established policies and guidelines. Influences care by collaborating with members of other team.</p> <ul> <li>Conduct site visits to the contract nursing homes; audit charting, interview Veterans regarding their care, ensure quality care is being delivered. </li><li>Identify patient care needs in accordance with the established standards, policies, and procedures consistent with nursing practice. </li><li>Interacts with Veterans, family members, and an interdisciplinary team of professionals. </li><li>Coordinates nursing care for Veterans with specialized/complex needs in the nursing home. </li><li>Be resource for questions and concerns related to the process for obtaining specialty care, therapy service, and equipment. </li><li>Assist with notifications related to sentinel and adverse events. </li><li>RN must follow up on referrals, consults, medical needs, and other tests to ensure continuity of care for Veterans residing in CNH homes. </li><li>RN must monitor for appropriate utilization of care by conducting routine inspections of community nursing homes that have a contract with the VA. </li><li>RN must be able to identify gaps in the care plan or provision of care and utilize available resources to fill the gaps in care. </li><li>The RN is responsible to ensure that the highest possible quality of care is achieved and that a safe therapeutic environment is provided. </li><li>The RN may need to develop creative and innovative strategies to resolve problems and/or </li></ul> <p>promote positive outcomes in collaboration with other CNH team members.</p> <ul> <li>The RN must document CNH visit notes and other case management needs per VA guidelines. </li><li>The RN must have an active driver's license and be able to sustain long periods of sitting and </li></ul> <p>driving. and may be exposed to weather such as tornadoes, rain storms, snow, ice, and other</p> <p>various weather related elements.</p> <ul> <li>The incumbent is responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans. </li><li>Other duties might be assigned </li></ul> <p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards</p> <p>Pay: Competitive salary, regular salary increases, potential for performance awards</p> <p>Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Licensure: 1 full and unrestricted license from any US State or territory</p> <p>Work Schedule: 0730 AM- 0400PM. Monday to Friday</p> <p>Telework: Yes, Ad-hoc.</p> <p>Virtual: This is not a virtual position</p> <p>Relocation/Recruitment Incentives: May be authorized</p> <p>Travel Required: Yes, 50%</p> <p>Preferred Experience: Candidate must have BSN. Minimum 2 years experience in case management, PACT, short/long term care.</p>
