We will train you on all health education topics!</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES:</p> <ul> <li> <p>Provide health education and care coordination support to patients</p> </li><li> <p>Help patients set a health goal and make small steps to work towards it</p> </li><li> <p>Help patients understand and navigate the health care system</p> </li><li> <p>Assist patients with applying to and accessing assistance programs/community resources</p> </li><li> <p>Build trusting relationships with patients</p> </li><li> <p>Facilitate Group Visits for patient education</p> </li><li> <p>Additional duties as assigned</p> </li></ul> <p>POSITION QUALIFICATIONS:</p> <p>Education and Experience:</p> <ul> <li>High school diploma or GED. </li><li>1-2 years' experience working in a community health setting or with underserved populations desired. </li><li>BA/BS degree in Health/Human Services field in lieu of experience accepted. </li><li>Experience providing health education and/or patient navigation desired* </li><li>Training provided upon hire </li></ul> <p>Knowledge, Skills and Abilities:</p> <ul> <li>Excellent verbal and written communication skills in English and Spanish required </li><li>Excellent organizational skills </li><li>Ability to flourish in an unpredictable daily flow within a team-based care model </li><li>Ability to proactively collaborate with an interdisciplinary healthcare team, internal and external to the clinic </li><li>Sensitivity to low income, ethnic minority communities </li><li>Strong critical thinking and problem-solving skills </li><li>Self-motivated with a demonstrated ability to take initiative </li></ul> <p>We are committed to equal employment opportunity regardless of race, color, ancestry, religion, sex, national origin, sexual orientation, age, citizenship, marital status, disability, gender, gender identity or expression, or veteran status.
