<p>Position Title: Home Health Care Registered Nurse (RN)</p> <p>Department: Home Health Services</p> <p>Supervisor: Chief Home Health Officer</p> <p>Status/Hours Per Week: Non-exempt / Full-time (30-40 hours/wk)</p> <p>Pay Range: The expected base pay for the position is $36-$65.02 per hour and may be increased based on other factors, such as language, certifications, etc.</p> <p>Primary Function:</p> <p>The Upham's Home Health Services has been in operation since 1975 serving thousands of Boston residents. Upham's Community Care is the only Boston health center to have a home health care and visiting nurse program.</p> <p>The Home Care Nurse conducts home visits to patients for evaluation, provides direct skilled nursing care, executes physician's orders as required, and is professionally responsible to the nursing supervisor.</p> <p>Duties & Responsibilities:</p> <ul> <li>Completes initial evaluation visits, including history and physical exam. </li><li>Regularly evaluates the patient's nursing needs. </li><li>Develops nursing care plan and makes necessary revisions. </li><li>Provides health services requiring substantial specialized nursing skills including appropriate preventative and rehabilitative nursing procedures. </li><li>Utilizes other community agencies and resources to provide comprehensive patient care. </li><li>Coordinates additional health services provided to patient. </li><li>Discuss changes in the patients' condition and needs with the physician and other appropriate personnel. </li><li>Supports the patients' family in meeting the physical, psychosocial and other health needs of the patient. </li><li>Oversees work of Home Health Aides as required in the patient's home. </li><li>Provides community consultation and health education when appropriate. </li><li>Keeps accurate records of patient's progress. </li><li>Has overall responsibility for maintaining their patient's records according to Home Health Services' protocol. </li><li>Participates in the regular case review meetings. </li><li>Participates in the weekend/holiday rotation. </li><li>Must provide a minimum of 25-30 patient encounters per week for a full-time position. </li></ul>
