<p>Current vacancies are in Family Health (Plato) and Clinical Services (Cedar) in the Public Health Department and Aging & Disability Services | MnCHOICES Division in the Social Services Department. The eligibility list created by this vacancy may be used to hire other vacancies in the department or county. </p> <p>Family Health Public Health Nurse (PHN) Duties in Family Health involve primarily working with pregnant and parenting families who may be experiencing significant life stressors to provide support and education to improve birth outcomes, assess child growth and development, develop positive parenting skills, and assist with making linkages to community resources. Public Health Nurses make home visits and/or provide community outreach and engagement to provide a full range of public health nursing services, referral, case management and community health services to individuals, families and the community. Experience working within diverse communities, speaking second language and with interpreters is desired. Job responsibility includes maintaining timely and accurate health/visit records using an electronic health record for documentation. Public Health Nurses are asked to assist in other areas of the Public Health Department, including responding to Public Health Emergencies that arise, one of the most recent examples of this work done by Public Health Nurses in the COVID-19 response. During employee orientation time, new employees to the programs may need to travel out of town for up to one week's time in order to obtain proper training.</p> <p>One of the PHN positions is working with the evidenced based home visiting model known as Maternal Early Childhood Sustained Home-visiting (MECSH) Model. MECSH is a high-quality evidenced based nurse-led structured intervention for families needing extra support. MECSH is a child focused prevention model uniquely embedded within a universal child and family health service system. </p> <p>The second PHN position is with the Teen Parent Program, the PHN provides home visiting services to pregnant and parenting teens, teens that are attending high school and on Minnesota Family Investment Program(MFIP). The PHN works with the teen to adhere to the MFIP requirements, promote self-sufficiency, including high school graduation/GED completion. </p> <p> </p> <p>Sexual Health Program (Clinic 555 Outreach) Public Health Nurse (PHN) This position works on a collaborative team of PHNs, Health Educators, MAs, LPNs and Providers (NPs and MDs) providing sexual health and harm reduction services to the community of Ramsey County in a variety of settings. These may include Mobile CareVan, on-site Clinic, encampments, shelters, treatment centers, schools, correctional facilities, or community events. Some of the duties of the PHN may include, patient assessments, asymptomatic STI testing, STI treatment, HIV testing and referrals for treatment, birth control education, comprehensive sexual health education, and naloxone training. Additional duties may include, working in our Syringe Services Program (SSP) providing safer use supplies, naloxone, wound care, education and referrals for a variety of people including people who inject drugs (PWID).</p> <p>Work Hours: 8:00am - 4:30pm, including requirement of one evening shift a week (either T or Th until 7pm), and some other evenings and weekends as needed.</p> <p>Aging & Disability Services | MnCHOICES Division Public Health Nurse (PHN) This position will be providing screening and face to face assessment services using the new MnCHOICES web-based tool for people who are aging or have disabilities. This assessment will determine eligibility of Elderly Waiver (EW) and Alternative Care (AC), Community Alternatives for Disabled Individuals (CADI), Community Alternative Care (CAC)Brain Injury (BI), Developmental Disability (DD), Rule 185 services, and Community First Services and Supports (CFSS). Develop a Community Support Plan. Document assessment and referral activities in SSIS. Identify potential support services and community resources based on assessed support needs. Complete county and state forms related to assessment activity. Make support service recommendations based on assessment results using a strength-based approach, to help people stay in their homes or in their community safely and as independently as possible. The Public Health Nurse will provide consultation to staff and collogues regarding assessment outcomes including those with multiple chronic conditions or complex health issues. And perform other related duties as assigned. A multidisciplinary approach is required, and qualified candidates will be selected based on creating a team with varying expertise. </p> <p>The Aging & Disability Services | MnCHOICES Division within the Social Services Department is responsible for administering Home and Community Based Services. The division is comprised of three major functional areas: The MnCHOICES unit, Administration & Access unit, and the Case Management unit. To view Ramsey County's Flexible Workplace policy, go to: Flexible Workplace Policy</p> <ul> <li>Perform independent health assessments of individuals, families and communities by evaluating their physical, mental, social and environmental health status; and provide nursing and community health intervention based on assessment of need and current community resources. </li><li>Make skilled nursing observations of client health status by assessing, monitoring and documenting such things as pregnancy, postpartum and infant/toddler status; developmental screening; depression screening; and evaluating the safety of home environments. </li><li>Share with clients, families, and communities methods of preventative positive health practices, such as health pregnancy; home and family safety; child growth and development; child spacing; and healthy parenting. </li><li>Provide case management for clients by developing care plans, consulting with other health care providers such as physicians and social workers, refer clients to other health care resources. </li><li>Provide orientation and training to Public Health Nurses, Health Educators, Dieticians, Home Health Aides, Health Education Program Assistants; and Public Health Licensed Practical Nurses by providing leadership in the area of case management, current medical and nursing technologies, current public health trends, and community resources. </li><li>Intervene in emergency situations by notifying proper authorities such as police, physicians, child protection agencies and personnel in medical facilities/emergency rooms, provide information to court officials in cases of child and adult protection. </li><li>Develop and provide health education and consultation services to community members, clients and family members, other professionals and Ramsey County employees on a one-to-one and group basis through home visits, clinics, health promotion and community outreach activities, respond to requests for information by phone or in person, coordinate and conduct health fairs, develop written and oral educational materials and curricula, and teach classes on topics related to public health issues, disease prevention and recognition, and safety and injury prevention, facilitate youth at-risk support groups on public health issues at local schools. </li><li>Serve on or chair internal and external work groups, task forces and advisory committees for the purpose of influencing and developing policy and procedures related to various community public health issues. </li><li>Assure privacy of client data according to federal and state regulations. </li><li>Maintain data related to activities and draft periodic reports to grantors as required. </li><li>Document cases, health promotion and community activities by preparing reports and necessary paperwork, develop and maintain required records and write case histories. Comply with child protection reporting requirements and collaborate with health care providers and community resources engaged with the family and schools. </li><li>Participate in team meetings and one-on-one meetings with supervisor. </li><li>Maintain data in a timely manner related to all activities as required. </li></ul> <p>(The work assigned to a position in this classification may not include all possible tasks in this description and does not limit the assignment of any additional tasks in this classification. Must have a valid driver's license. Examination/Screening Process: The examination for Public Health Nurse will consist of a training and experience rating, comprised of the questions in the attached supplemental questionnaire. The rating on this supplemental questionnaire will depend on the answers provided. A candidate must pass the training and experience rating in order to be advanced in the process.</p> <p>Eligible List: The names of all applicants who properly completed an application and passed the examination/screening process shall be placed on the eligible list for an employment opportunity as Public Health Nurse. This list will be certified to the appointing authority which may use this list to conduct interviews to fill a vacancy. Candidates will remain on the list for 90 days or until hired, whichever occurs first. A notice will be sent to applicants at the time the eligible list is posted, informing them that the list has been certified and their status. The department may choose to interview any candidate on the eligible list.</p> <p>Veterans' Preference: This is a classified position requiring an open, competitive selection process.
