We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.</p> <p>Full Time 40 Hours</p> <p>Sign on Bonus! $3000 for eligible LCSW non-MGB employee available, please ask for more details.</p> <p>Job Duties and Responsibilities:</p> <p>60% Access and Care Coordination:</p> <p>Facilitate needs for patients to the primary care psychiatry clinics. This can include but is not limited to:</p> <p>Assist patients who do not qualify for care within MGH in navigating community resources accessible to them based on insurance, geographical location, and access to transportation</p> <p>Engage family supports when clinically appropriate</p> <p>Identify potential additional services from which the patient may benefit - day treatment, support groups, VNA</p> <p>Facilitate referrals to higher levels of care i.e. Partial Hospitalization or Intensive Outpatient programming when needed</p> <p>Assist patients needing assistance with transportation to the clinic, fuel and utility assistance, navigation of housing resources, connection to legal counsel, applications for Disability or the Department of Mental Health</p> <p>Work with other department social workers and resource specialists, assist patient transitions from the clinic to community-based providers when deemed indicated. This can include but is not limited to: referrals for community therapists, support groups, career services, day treatment programs, visiting nurses or PCAs, connection to legal counsel, financial supports such as discounted phone service, discounted food and utilities, and transportation assistance. Facilitate applications for Social Security, Disability and the Department of Mental Health as well as assist in navigating housing resources.</p> <p>Refer patients to specialty clinics within MGH for consultation or ongoing care when appropriate</p> <p>Support patients via phone contact in adhering to behavioral management plans when outlined, or in following through with upcoming appointments</p> <p>Coordinate care with any other existing internal or external providers</p> <p>Clinical 40%</p> <p>Perform intake evaluation assessment of patients seeking therapy to the clinic to determine appropriate care-this may be performed through a brief phone assessment or full clinical intake assessment</p> <p>Manage patient calls that require an on-the-spot clinical assessment of the severity of a patient's symptoms and/or potential threat of harm to self or others, and implement appropriate interventions</p> <p>Provide support and case management as part of clinical treatment team to patients at high-risk or with acute needs</p> <p>Provide psychoeducation to patients/families regarding illness, highlighting protective factors and working together define adaptive coping strategies</p> <p>Provide time limited solutions-focused with a small caseload of patients. This can include but is not limited to:</p> <p>Supportive counseling to patients experiencing psychosocial stress, loss and/or grief, interpersonal or relationship difficulties, or adjustment to illness</p> <p>Work with patients around identifying and practicing adaptive coping mechanisms</p> <p>Operate flexibly within the treatment relationship and utilize various frameworks and modalities including incorporating a psychodynamic perspective, or implementing CBT and DBT techniques when clinically indicated</p> <p>Job Summary</p> <p>Working within a multidisciplinary team of psychiatrists, psychologists, and patient service coordinators, the Outpatient Psychiatry Social Worker is responsible for providing clinical support and case management to the Outpatient Psychiatry Department. The Social Worker will work alongside the department's social work teams to appropriately triage patient needs, perform clinically informed assessments, complete time-limited therapy, and connect patients with community-based resources. This position requires a high degree of flexibility and willingness to participate in program planning as this role within the department evolves.</p> <p>Qualifications</p> <p>Education</p> <p>Master's of Social Work (MSW) from an accredited program required.</p> <p>Can this role accept experience in lieu of a degree? No</p> <p>Licenses and Credentials</p> <p>Massachusetts Licensed Independent Clinical Social Worker (LICSW) required.</p> <p>Licensed Clinical Social Worker (LCSW) with relevant clinical experience.</p> <p>Experience</p> <p>Clinical experience in medical setting 0-1 year required.</p> <p>Knowledge, Skills and Abilities</p> <ul> <li>Excellent organizational and time management skills. </li><li>Excellent crisis intervention skills. </li><li>Good problem solving and conflict resolution skills. </li><li>Ability to work well collaboratively and independently. </li><li>Strong written and verbal communication skills. </li><li>Knowledge of community resources and the aging process. </li><li>Demonstrates basic foundational skills, showing fundamental knowledge and a commitment to the application of professional values and ethics. </li><li>Demonstrates fundamental skills in formulation, assessment of risks, crisis intervention completion of psychosocial treatment plans, and appropriate documentation. </li><li>May need assistance and guidance in ensuring the patient's needs, safety measures, and concerns are brought forward. Emerging knowledge of internal and external resources. </li><li>Developing and demonstrating interpersonal collaboration in a medical setting- inpatient or outpatient. </li><li>May need coaching/guidance in this area. </li><li>May provide some assistance and support with onboarding for new team members. </li><li>Aware of opportunities in department/service initiatives; participates in initiatives at an entity level. Uses data to measure progress. </li><li>Demonstrate fundamental skills under supervision.
