You'll design and manage best-in-class intake processes, mentor a high-performing team, and collaborate across departments to elevate our referral experience to industry-leading standards.</p> <p>What You'll Do</p> <ul> <li> <p>Develop and oversee efficient, scalable intake workflows for home health and hospice referrals from hospitals, physicians, community partners, and internal teams.</p> </li><li> <p>Ensure all patient data is accurate and complete upon intake, eliminating errors that impact care or reimbursement.</p> </li><li> <p>Lead and mentor the Intake team, fostering a culture of excellence, education, and accountability.</p> </li><li> <p>Use data to drive decisions, improve throughput, and reduce delays in starting care.</p> </li><li> <p>Maintain strict compliance with HIPAA and payer-specific eligibility requirements.</p> </li><li> <p>Partner with referral sources to streamline submission processes and enhance their experience.</p> </li><li> <p>Collaborate with Marketing and Revenue Cycle teams to align intake operations with sales strategies and billing insights.</p> </li><li> <p>Contribute to special projects and initiatives that support agency growth and operational excellence.</p> </li></ul> <p>The Must Haves</p> <ul> <li> <p>Bachelor's degree in Healthcare Administration, Business, or related field.</p> </li><li> <p>5+ years of progressive leadership experience in a high-volume intake or admissions role within home health or hospice.</p> </li><li> <p>Proven success in optimizing referral processes across multiple channels.</p> </li><li> <p>Deep knowledge of insurance verification, documentation, and authorization requirements.</p> </li><li> <p>Strong organizational, communication, and analytical skills.</p> </li><li> <p>Proficiency in EHR systems and CRM tools used in post-acute care.</p> </li></ul> <p>About Us</p> <p>Synergy Shared Services is a newly formed company in the Pennant family.
