<p>Providing Care Coordination & Outreach Services to Increase</p> <p>The member will provide MSM with the sustainability of funding, volunteer staffing and ability to deliver high-quality healthcare to low income and uninsured populations, including patients who are houseless and on the street. The member will either finish the WAFCC Silver Standards of Excellence or develop a process for achieving WAFCC's Gold Standards of Excellence, enhance MSM's internal and external communications systems, expand MSM's resource development system, and engage with other capacity building projects. The member will build the capacity of MSM to deliver high-quality healthcare to the most vulnerable populations, ultimately enabling patients to break the cycle of poverty by providing access to affordable health coverage. Further help on this page can be found by clicking here.</p> <p>Member Duties : ¿ Provide follow-up with clients following clinics these might include, helping clients set up primary care appointments, giving them supplies that are not available at the clinic, and checking in to see if they followed through with next steps. ¿ Set up cab rides for clients to reach various appointments/procedures ¿ Engage with clients to remind them of appointments and make a plan for keeping them ¿ Travel to local pharmacies to pick up medications for clients and delivering them, or handing off to other staff for handoff ¿ Provide basic supplies to clients on street outreach (toiletries, food, water, etc.) ¿ Perform exploratory outreach to locate and identify areas where clients stay Member will assist case managers and lived experience workers focusing on the following: ¿ Provide clothing and basic supplies (toiletries food, water, etc.) to client walk-ins during office hours ¿ Support outreach staff</p> <p>Program Benefits : Health Coverage , Education award upon successful completion of service , Training , Stipend , Childcare assistance if eligible , Living Allowance .</p> <p>Terms :</p> <p>Car recommended , Permits working at another job during off hours , Permits attendance at school during off hours .</p> <p>Service Areas :</p> <p>Homelessness , Community Outreach , Housing , Education , Health , Community and Economic Development .</p> <p>Skills :</p> <p>Public Health , Education , Conflict Resolution , Counseling , Writing/Editing , Communications , General Skills , Community Organization .</p>
