<p>Position Summary:</p> <p>The Summit SBHC Patient Access Representative is the first point of contact between the students and their families and our School Based Health Center with responsibilities for supporting the students and their families either in person or via the phone, offering a professional invitation into our clinic while upholding the ASQ (Access, Stewardship, and Quality) pillars. Maximizing patient scheduling opportunities with attention to operational flow and the needs of the patient. </li><li>Screen all new and established patients for eligibility for all School Based Health Services. </li><li>Patient Appointment Preparation: Prepare patient records for appointment readiness. Gather required paperwork and update demographics, insurance, enrollment for accurate records completion in the Electronic Records system. </li><li>Responsible for filling schedules, scrubbing, and reminder calls for Summit High, Summit Middle, Dillon Valley Elementary, & Silverthorne Elementary School. </li><li>Collect payment and complete end of day reconciliation. </li><li>Participate in school and community events including, but not limited to, back to school nights, school registrations, and SBHC immunization clinics. </li><li>Lead and participate in team huddles. </li><li>Provide administrative support for integrated care. </li><li>Other duties as assigned by SBHC manager. </li></ul> <p>Additional Requirements:</p> <ul> <li>All jobs at the Care Clinic are impacted by various grants which are necessary for our sustainability. The environment is primarily clinical, but the individual may work in a clerical setting. Many of our patients with chronic disease have a minimal level of health literacy and a significant number of barriers to accessing health care.
