Serving as the primary point of contact for the program, this role is responsible for ensuring the logistical success of the mobile trauma-informed primary care service. Street medicine is a mobile trauma-informed primary care and outreach service conducted in partnership with several local agencies and with learners involved in the care.</p> <p>Minimum Qualifications</p> <ul> <li>Bachelor's degree in a related health, allied health, social science area, business, or related field and 3 years of relevant experience or associate's degree with 5 years relevant experience. </li><li>Relevant experience should be in nursing, social work, public health, healthcare setting, training/mentoring, or in leadership capacity. </li><li>Experience working with individuals who may be experiencing homelessness, mental illness, substance use disorder, or comparable experience. This experience can include experience gained through intern/externships, volunteering, and/or student employment. </li><li>Current Basic Life Support (BLS) CPR certification with AED training is required at the time of hire. </li><li>Ability to manage multiple responsibilities in a dynamic and fast-paced environment, including logistical planning, clinical coordination, and community outreach. </li><li>Highly organized individual with excellent oral and written communication skills. </li><li>Current valid Ohio state driver's license or ability to travel within the Montgomery County region. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Medical Assistant Certification </li><li>Experience in street medicine, mobile healthcare, or community health outreach is highly preferred. </li></ul> <p>Essential Functions and percent of time:</p> <p>Program Coordination and Community Partnership Management - 60%: The Program Coordinator is responsible for the seamless integration of program management and coordination with community partners, ensuring that both clinical and outreach operations function efficiently. The role involves extensive relationship-building, planning, and execution of outreach events, case management, and quality improvement processes.</p> <ul> <li>Lead the scheduling and logistics for the street medicine unit, including coordination with healthcare providers, social service agencies, and community partners (e.g., Premier Health, the Dayton Dream Center, Public Health). </li><li>Develop and implement program policies and procedures in collaboration with physicians, leadership, and partner organizations. </li><li>Oversee the integration and coordination of street medicine services with partner organizations, ensuring that services are communicated and available to the target population. </li><li>Provide administrative support for the program, including tracking program metrics, assisting with grant reporting, and managing budget and resource allocation. </li><li>Coordinate the participation of medical students, residents, and other learners, providing supervision, teaching, and mentorship during patient interactions and outreach events. </li><li>Ensure operational readiness for outreach events, including the set-up, staffing, and execution of clinics at partner sites, as well as providing on-site support as needed. </li><li>Serve as the primary point of contact for scheduling and coordinating care, referrals, and patient follow-up. </li><li>Promote and advertise clinic services at partner sites, working with staff to ensure effective communication about program offerings. </li><li>Develop and maintain strong relationships with community agencies, stakeholders, and funding organizations, ensuring effective partnerships that support the program's sustainability and growth. </li><li>Oversee the case management of patients, providing social service connections and facilitating continuity of care for individuals with complex health and social needs. </li><li>Lead program evaluation efforts, participating in quality improvement activities and care reflection sessions to enhance the program's effectiveness and patient outcomes. </li><li>Document patient interactions, coordinate reporting to funders, and ensure compliance with all program requirements. </li><li>Participate in outreach efforts and public events to engage the community and build awareness of the street medicine program. </li><li>Perform other related duties as directed. </li></ul> <p>Essential Functions and percent of time (cont'd):</p> <p>Clinical Responsibilities-40%: The Program Coordinator works closely with physicians and other healthcare providers to ensure optimal care delivery, while also leading and supervising students.</p> <ul> <li>Support clinical team members, including medical students, residents, and other healthcare providers, in the provision of trauma-informed primary care to patients. </li><li>Perform pre-visit planning activities and facilitate the pre-visit huddles. </li><li>Support physicians by assisting with patient care, managing treatment procedures, and ensuring proper documentation in electronic medical records. </li><li>Conduct detailed medical and social history interviews, record vital signs, and contribute to care plans for patients with complex medical and social needs. </li><li>Monitor clinical supplies and equipment to ensure readiness and safety for patient care, including clinical supply stocking, checking expiration dates, etc. </li><li>Maintain safety and cleanliness of examination/ treatment rooms and laboratory areas. </li><li>Act as a clinical point of contact for students and learners, providing mentorship, guidance, and supervision in the clinical setting. </li><li>Collaborate with physicians to phone in prescriptions, schedule follow-up care, and ensure proper referral processes for specialty care. </li><li>Provide education on preventive care and health maintenance, including reproductive health, substance use prevention, and immunizations. </li><li>Participate in emergency response training and ensure readiness for acute care situations within the street medicine environment. </li><li>Collaborate with leadership to establish clinical protocols and ensure compliance with health regulations. </li><li>Perform other clinical tasks as assigned. </li></ul> <p>Non-Essential Functions and percent of time: Working Conditions</p> <p>The work environment and schedule include at least one clinic half a day per week in a mobile setting.
