<p>Job Summary:</p> <p>Acts as a central point of contact in the surgical practice, guiding patients through their entire surgical journey from pre-operative planning through recovery. This role is crucial for improving care coordination, educating patients, and removing barriers to treatment. PCNs are individuals with excellent communication and interpersonal skills who establish trusting professional relationships with patients. The PCN, under the general direction of the office manager, provides patient and practice support to enhance patient engagement in meeting care plan goals. The PCN works with the practice to close care gaps and works closely with the surgical teams to identify and remove barriers to care.</p> <p>Duties and Responsibilities:</p> <p>Collaborates with the interdisciplinary team in guiding the patient and family regarding diagnostic and treatment options. Assumes the role of care coordinator and patient advocate throughout the process.</p> <p>Coordinates tests, treatments, referrals, and appointments as necessary</p> <p>Screen new patient referrals and ensure appropriate testing has been completed prior to visit, help coordinate imaging retrieval and ensure they are uploaded for visit</p> <p>Coordinate tumor/nodule board including documentation and patient follow up</p> <p>Ensure patients complete pre-operative work up and clearance and that post op orders are in place</p> <p>Coordinate with OR and office schedulers to optimize patient throughput and new patient scheduling</p> <p>Post operative contact with patients</p> <p>Facilitating communication between patients, their families, referring physicians, surgeons, and other members of the multidisciplinary care team.</p> <p>The navigator ensures all necessary pre-operative workups are completed and helps coordinate care between various specialists and departments.
