<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Day Shift</p> <p>Description:</p> <p>Registered Nurse-Liaison-Hospice-Intake-Albany</p> <p>The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as a Liaison in local hospitals in the Capital Region-primarily working at Albany Medical Center.</p> <p>St. Peter's Health Partners Highlights:</p> <ul> <li>Mileage Reimbursement </li><li>Quality of Life: Where career opportunities and quality of life converge </li><li>Advancement: Strong orientation program, generous tuition allowance and career development </li><li>Work/Life: Positions and shifts to accommodate all schedules </li></ul> <p>What you will do:</p> <ul> <li>Evaluate patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care. </li><li>Coordinate with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services. </li><li>Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services. </li><li>Admit patients to Hospice in the hospital setting upon eligibility. </li><li>Work closely with the hospital case management department. </li><li>The work schedule is flexible, generally weekdays (M-F) between 8am to 4pm. Weekend opportunities may also exist from time to time. </li></ul> <p>What you will need:</p> <ul> <li>A current license to practice as a Registered Nurse in the State of New York </li><li>Associates Degree in Nursing, BSN preferred </li><li>1 year experience as an RN required, preferably in Hospice/Home Care or acute care. </li><li>Hospice or RN Liaison experience a plus but not required. </li><li>The RN must be able to communicate effectively, verbally and written </li><li>Valid Driver's License and Reliable vehicle </li><li>High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers </li><li>Understanding of Hospice principles preferred </li></ul> <p>Training provided in end of life care.</p> <p>Community Hospice provides a thorough orientation, preceptors and a supportive work environment.</p> <p>Apply today for more information.</p> <p>Pay Range: $35.00-$47.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
