<p>Job Summary</p> <p>The Endocrinology Nurse Navigator serves as a consistent care coordinator, ensuring timely access and seamless coordination of services throughout the continuum of care. This role utilizes a focused and proactive approach in guiding patients and families through complex endocrine conditions, treatment plans, and long-term disease management. The Nurse Navigator educates patients, families, and caregivers regarding disease processes and the importance of close, continuous monitoring. Acting as a bridge between professional staff and patients, the navigator demonstrates leadership, initiative, and teamwork while building collaborative relationships that enhance the patient experience and improve outcomes. This position works closely with multidisciplinary teams, specialty navigators, and data registries to ensure quality-driven, coordinated care delivery.</p> <p>Responsibilities</p> <p>Care Coordination & Management</p> <ul> <li>Cost-effectively manage patients across the entire continuum of care. </li><li>Coordinate patient care across all healthcare settings and providers. </li><li>Interface with attending and consulting physicians, residents, nurses, and multidisciplinary teams to identify and resolve system barriers to timely care. </li><li>Develop, implement, and evaluate clinical practice guidelines and care pathways. </li><li>Collaborate with internal and external resources to develop and maintain telemedicine and/or remote monitoring networks. </li></ul> <p>Patient Education & Support</p> <ul> <li>Provide comprehensive education to patients and families regarding endocrine disease processes, treatments, and self-management strategies. </li><li>Foster patient engagement and self-care activities to ensure high-quality outcomes. </li><li>Coordinate referrals and community resources to optimize patient outcomes in a cost-effective manner. </li><li>Lead the development of standards for patient education. </li></ul> <p>Quality & Performance Improvement</p> <ul> <li>Monitor clinical, quality, and economic outcomes for the endocrine patient population. </li><li>Collect and evaluate patient data to support ongoing quality improvement initiatives. </li><li>Develop quality filters, audits, and case reviews to identify trends and implement corrective actions. </li><li>Participate actively in committees related to prevention, screening, and specialty care initiatives. </li><li>Maintain knowledge of ongoing clinical research activities. </li><li>Contribute to performance improvement initiatives and identification of system opportunities. </li></ul> <p>Leadership & Collaboration</p> <ul> <li>Demonstrate leadership by translating strategic priorities into operational execution. </li><li>Serve as a liaison between patients, caregivers, and healthcare teams. </li><li>Participate in tumor boards, specialty conferences, and interdisciplinary meetings as appropriate. </li><li>Drive program development, community outreach, prevention education, and health screening initiatives. </li></ul> <p>Department-Specific Responsibilities</p> <ul> <li>Collaborate with specialty navigators and clinical registries to ensure comprehensive care coordination. </li><li>Assist with staging, diagnostics, biomarker testing, and pre/post-treatment planning when applicable. </li><li>Participate in multidisciplinary case reviews and specialty board meetings. </li><li>Maintain specialty program scorecards and assist with data collection and follow-up activities. </li><li>Conduct post-treatment follow-up calls and ongoing patient check-ins. </li><li>Provide counseling support and connect patients with wellness resources as needed. </li></ul> <p>Knowledge, Skills & Abilities</p> <p>Patient Group Knowledge (Only applies to positions with direct patient contact)</p> <p>The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages.
