<p>Apply</p> <p>Job Type</p> <p>Full-time</p> <p>Description</p> <p>Purpose:</p> <p>To assist in the provision of a wide variety of person centered supports and services with a strong emphasis on personal choice and independence for people with developmental disabilities. Actively assist the Health Coordinator, Assistant Director, and Residential Manager with functions such as: medication administration, appropriate medication storage, medication inventory and ordering medications appropriately when needed, support Direct Support Professionals (DSP's) with nursing standards of care such as, bathing, repositioning, and dressings, and provide direct support to people receiving services.</p> <p>Responsibilities:</p> <ul> <li>Actively promote/influence/support team members regarding health/medical information daily on site. Seek opportunities to assist DSPs in matters related to health and nutrition. </li><li>Communicate and document medical/health information to health coordinators, people supported as appropriate, families, and staff in accordance with HIPAA standards </li><li>Compile information for the RN such as for Surrogate Decision-Making Committee and/or intake packets. </li><li>Assist with medical/health appointments to provide support and information, including attending the appointment if needed. </li><li>Visit hospitalized/ill person to offer support, obtain and communicate information to the RN. </li><li>Daily presence in the Residential Program. </li><li>Develop and maintain a positive, influential and supportive professional relationship with all parties. </li><li>Administer medications and provide direct support/care. </li><li>Pursue a deep knowledge of the electronic software that AIM uses to document contemporaneous notes and to assist others in the programs with data entry. </li><li>Seek/guide/and influence DSP's in their provision of "delegated health services." Assist in ensuring compliance with appropriate regulations, policies and procedures. </li><li>Audit and maintain health/medical department's files to ensure information is current (i.e. personal information sheet, nursing plans of care, etc). </li><li>Conduct Medication audits as requested, document results; assure action is taken to address deficiencies and report findings to the Nurse and Residential Manager. </li><li>Any and all other related duties as requested by supervisor. </li></ul> <p>Requirements</p> <p>Qualifications:</p> <ul> <li>Current/ Valid New York State Licensed Practical Nurse (LPN). </li><li>One-year post-licensing experience (nursing home, hospital, etc.). </li><li>Experience working with people with developmental disabilities and traumatic brain injuries preferred. </li><li>Current CPR Certification </li><li>Valid New York State driver's license. </li><li>Flexibility with schedule to meet individual needs and to positively influence, support and provide guidance to DSP's </li></ul> <p>Physical/Cognitive Requirements:</p> <ul> <li>Excellent verbal and written communication skills. </li><li>Organizational and time management skills. </li><li>Lifting/transferring a person/item of various weights (50lbs minimum). </li><li>Bending/reaching/pushing/pulling. </li></ul> <p>Reports To: Assistant Director of Residential and Health Care Coordinator (RN).</p> <p>Salary Description</p> <p>$27- $34</p>
