<p>CASE MANAGER</p> <p>HEAD START</p> <p>FULL-TIME (35/40 HOURS PER WEEK) PART YEAR</p> <p>GENERAL RESPONSIBILITY: Provide support to families in accessing community services as they work toward self-sufficiency goals and school readiness for their children.</p> <p>SPECIFIC RESPONSIBILITIES: In conjunction with families identify the family's social service needs, case management, recruitment, conducting self-sufficiency assessments and goal plans, maintaining data and statistics for the PIR, submitting monthly statistical reports, conducting home visits, maintaining client records, tracking attendance and appropriate follow-up, collaborating with colleagues and community partners, maintaining electronic records for all families. Assist with recruitment, selection, enrollment and attendance. Ensuring that all documentation and social service tracking is completed in a timely manner utilizing Child Plus System.</p> <p>QUALIFICATIONS: Bachelor's Degree in Social Work or closely related field required.</p> <p>SPECIAL QUALIFICATIONS: Strong self-starting skills, compassion for clients, work cooperatively in a team environment, and understand children and family's special circumstances. Must have a valid driver's license, reliable transportation, and vehicle insurance. Demonstrate an understanding of everyone's experiences, every day.
