Our phase-based care model will help people thrive by continuously revolutionizing healthcare delivery, using a data-driven and team-based approach.</p> <p>Brief Description of job sites:</p> <p>The Recovery Collaborative Care team provides immediate, non-crisis substance use and recovery support and therapeutic services for adults 18 and older. This includes Indigent clients and clients with Medicaid or Kaiser insurance. The Recovery Clinic team provides wraparound clinical services from care coordination, initial and concurrent assessments, individual therapy, case management and medication management services. The Recovery Clinic provides services both in-person and virtually Monday through Thursday.</p> <p>The Mobile MAT program provides access to MAT for treating opioid use disorders for people who live outside of metro Denver. In addition, it provides connections to Jefferson Center services including senior services, wellness services, and community outreach programs such as suicide prevention trainings and Mental Health First Aid. All while seeking to offer our community access to the best behavioral health, substance use, and wellness services starting on the same day they are requested, without exception.</p> <p>Essential Duties:</p> <p>For this role, the responsibilities of a Nurse Practitioner include but are not limited to:</p> <ul> <li>Provides diagnostic/medication evaluations, and individual and group medication management; provide medication education and psychoeducation </li><li>Consults to nurses triaging phone calls to address medication problems and refill requests </li><li>Orders and monitors appropriate laboratory tests at clinically indicated frequency </li><li>Monitors and reviews consumers' charts for medication/medical issues with appropriate follow-up </li><li>Administer evidence-based measurement tools regularly and consume outcome data to drive clinical collaboration discussions and consequently, a client's care planning </li><li>Identify risk factors such as lethality for suicidal, homicidal, and/or grave disability and coordinate with emergency services as necessary </li><li>Participate in evidence-based practices and other agency initiatives for mental health and co-occurring substance use services </li><li>Exhibit enthusiasm, courtesy, adaptability, and spirit of cooperation and teamwork in the work environment </li><li>Effectively respond to client needs and problems; initiate and maintain positive interactions; timely response to messages and other requests </li><li>Utilize data and new operational practices to better adapt caseload complexity </li><li>Timely documentation in the electronic health record </li><li>Coordination for emergency services when clinically indicated or medically necessary; risk/lethality assessment </li><li>Consistently meets the expectations of the job </li></ul> <p>Note: Employees are held accountable for all duties of this job. This job description is not intended to be an exhaustive list of all duties, responsibilities, or qualifications associated with the job.</p> <p>Education, Knowledge, Skills and Experience Required:</p> <ul> <li>RN degree (or equivalent), Master's Degree in Nursing, CNS (Clinical Nurse Specialist) certification, Prescriptive Authority licensure by the Colorado Board of Nursing, and DEA certificate </li><li>Knowledgeable to psychopharmacology, medical/psychiatric interface, and evidence-based neuropsychiatric/neurobehavioral diagnosis and treatment </li><li>Ability to collaborate with multidisciplinary team in the delivery of comprehensive and integrated interventions </li><li>Bilingual preferred but not required </li><li>Ability and willingness to work in face-to face client settings or via video telehealth </li><li>Skills, experience, or vested interest in integrated health care approaches </li><li>Comfort or knowledge with prescribing MAT (Medication Assisted Treatment) medications such as suboxone. </li><li>Comfort with working with indigent and individuals experience co-occurring disorders (including severe substance use and psychotic disorders). </li></ul> <p>NP Salary Range: $105,300 to $130,700*</p> <p>Nurse Practitioner Bonus:</p> <p>$2,000 one month anniversary</p> <p>$1,500 one year anniversary</p> <p>($3,500 total)</p> <p>Additional Salary & Bonus Information*:</p> <ul> <li>The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.* </li><li>Based on full time (40 hours/week).
