This nurse will serve as a care coordinator, educator, and liaison, and will identify and close potential gaps in care to provide efficient and coordinated care between departments. The Oncology Nurse Navigator provides education on treatment options, emotional support, social support, physical care, communication assistance, and collaborates with community agencies.</p> <p>Duties & Responsibilities</p> <ul> <li>Contacts patients at the time of cancer diagnosis and educates them, provides information to new and existing patients and families regarding diagnosis, work-up, and treatment (surgical, systemic, and/or radiation therapy). </li><li>Ensures the coordination of new patient referrals, procedures, and appointments. </li><li>Evaluates new patients with cancer for emotional, social, or other barriers to care and refers them to the appropriate resources. </li><li>Serves as an advocate for oncology patients and works along with physicians and other departments to obtain/expedite recommendations for timely and appropriate patient care. </li><li>Identifies potential gaps in the coordination of patient care on a programmatic level, including moving from inpatient services to outpatient services, or referrals to tertiary care. </li><li>Coordinates and provides community education on screening and the early detection of disease specific cancers in collaboration with outreach programs. </li><li>Breast cancer nurse navigator - Coordinates financial assistance with mammograms and further diagnostic testing for patients that meet eligibility criteria. </li><li>Participates in multidisciplinary consults and tumor conferences to identify care opportunities that support complex patient needs, incorporating NCCN guidelines for site specific and supportive care, and prepares concise comprehensive case summaries. </li><li>Maintains documentation and a database of patients referred to Navigation Services. </li><li>Remains current in the latest information on changes in Cancer care and the educational material available. </li><li>Assists with development of policies, procedures, and standards reflective of current practice/literature. </li><li>Serves as an advocate within the community for awareness, prevention, and a champion of community oncology resources/services. </li><li>Participate in quality and performance improvement activities, assisting in data collection for meeting ACOS CoC standards, reporting pertinent data to the Cancer Committee with the goal of improving the standard of care for cancer patients. </li></ul> <p>Qualifications</p> <ul> <li>BSN degree required. </li><li>Current licensure to practice as a Registered Nurse in SC. </li><li>Oncology experience required. </li><li>Will be required to achieve OCN or certification in the appropriate field within two years. </li><li>Proficient in office computer software use. </li><li>Exceptional communication and customer service skills. </li></ul> <p>Benefits*</p> <ul> <li>Medical Insurance & Wellness Offerings. </li><li>Compensation, Retirement & Financial Planning. </li><li>Free Financial Counseling. </li><li>Work-Life Balance & Paid Time Off (PTO). </li><li>Professional Development. </li><li>For more information, please visit: anmed.org/careers/benefits </li><li>Varied benefits packages are available for positions with a 0.6 FTE or higher. </li></ul>
